Unveiling the link: Evaluating MTHFR gene polymorphisms and colorectal cancer risk through meta-analysis

Colorectal cancer pathogenesis is a multifactorial process, with genetic factors playing a significant role in cancer development. A review of published meta-analyses on MTHFR gene polymorphisms and colorectal cancer susceptibility showed inconsistent findings and failed to assess the reliability of statistically significant results. Case-control studies were manually searched in databases to investigate the association between MTHFR gene polymorphisms and colorectal cancer. The study assessed the strength of association for the five gene models by calculating odds ratios (ORs) and 95% confidence intervals (CIs). The study results were also analyzed for the source of heterogeneity, sensitivity, publication bias, and false-positive report probability (FPRP) test. Additionally, extensive subgroup analyses were conducted to investigate the impact of confounding factors on the associations. The study suggests that MTHFR C677T gene polymorphism reduces the risk of colorectal cancer in Asian and mixed-race populations, while increasing the risk of Colorectal cancer in the Indian ethnic group. MTHFR A1298C may play a protective role in the development of colorectal cancer. These findings provide valuable insights for the early diagnosis and prevention of Colorectal cancer. However, further studies are required to confirm the association, which may offer additional information for the early diagnosis and prevention of Colorectal cancer.

Nasal vs. oral BREATHing WIn Strategies in healthy individuals during cardiorespiratory Exercise testing (BreathWISE)

Background Nasal and oral exclusive breathing modes have benefits and drawbacks during submaximal exercise. It is less known whether these responses would extend to anaerobic work performed at high intensity. The purpose of this study is to find the most efficient mode of breathing during different phases of a maximal exercise at cardiopulmonary exercise test (CPET). Methods Healthy subjects were recruited to perform 4 maximal CPETs (standard conditions (STD), exclusively nasal breathing (eNAS), exclusively oral breathing (eOR), partial nasal breathing (pNAS) with just one blocked nostril) using the same ramp protocol on an electronically braked cycle ergometer. Before the exercise a standard spirometry was executed in the same order. Twelve healthy subjects (28.6 ± 5.2 y, 50% males) performed the 4 CPETs within one month. Variables were analysed at rest, at anaerobic threshold (AT), at intermediate exercise steps, and at peak. Results Compared to STD, eOR, and pNAS conditions, eNAS was associated with a significant lower peakVO2, peakVCO2, peak ventilation, respiratory rate, VE/VCO2 slope, respiratory exchange ratio, and workload (p < 0.05 for all). Moreover, peak inspiration and peak expiration time were augmented, while forced expiratory volume and vital capacity at rest were reduced. Only minor differences were detected at rest or AT. eNAS breathing Borg scale was higher in all phases of the exercise. Conclusions In young healthy subjects, an exclusively nasal respiration induces significant impairment on peak exercise capacity at CPET due to ventilatory limitation, with only minor effects on metabolic parameters at rest and in submaximal effort.

Walking cadence as a measure of activity intensity and impact on functional capacity for prefrail and frail older adults

Walking cadence has been suggested as a measure of activity intensity; however, it remains uncertain if prefrail and frail older adults can increase their walking cadence and if doing so leads to improvements in functional capacity. We aimed to determine if cadence can be increased and if this leads to improvement in functional capacity in prefrail and frail older adults. We performed a secondary data analysis of a walking intervention in prefrail and frail older adults living in retirement communities. Patients were randomized to Casual Speed Walking (CSW) and High-Intensity Walking (HIW) groups. Our primary outcome was improvement in 6-minute walk test distance above the minimally clinical important difference. We performed linear and logistic mixed-effects regressions to analyze our aims. 102 participants were included in the final analysis with 56 in the CSW group and 46 in the HIW group. Participants in the HIW group increased their walking cadence as compared to the CSW group during the intervention (HIW 100[88, 111] steps/min vs. CSW 77[65, 86] steps/min; P < 0.001). Participants that increased their walking cadence demonstrated an increased odds of improvement in their 6-minute walk test minimum clinically important difference (OR: 0.11, 95% CI: 0.033, 0.18; p = 0.005). Older adults can increase their walking cadence and walking cadence can serve as a surrogate measure of activity intensity during walking interventions. An increase of 14 steps/minute from their comfortable walking cadence increased the odds of improvement in 6-minute walk test minimum clinically important difference.

Stakeholder acceptance and attitudes toward dengue prevention: A study on fogging and autocidal traps techniques in Malaysia

The ineffectiveness of current prevention efforts is underscored by the increasing number of dengue cases, emphasizing the necessity of innovative approaches. This investigation investigates the extent of stakeholder acceptability and attitudes toward the alternative autocidal trap method and the current fogging technique. The objective is to identify differences in community viewpoints regarding these control techniques among various stakeholder groups, including scientists and the public. Information was collected from two separate groups of individuals: scientists and the general people. A comprehensive survey assessed the level of tolerance and attitudes towards fogging and autocidal traps. Statistical analysis was performed using the SPSS software, specifically utilizing the one-way MANOVA test, to identify any significant variations in attitudes among various stakeholder groups. The results reveal significant discrepancies in the perceived danger level between the two groups since the public views fogging as a riskier activity than scientists. However, the groups had no significant difference in the reported benefits. Scientists exhibited a greater degree of endorsement for both fogging and autocidal traps when compared to the public. While the general population showed more openness towards fogging, scientists have shown a more balanced approval of both tactics. These insights are essential for policymakers and health practitioners as they inform the creation of more efficient, community-oriented initiatives to address the increasing prevalence of dengue cases.

Surveillance of tuberculosis incidence and mortality through spatio-temporal analysis in Oyo State, Nigeria

The study aimed to describe the epidemiological profile of Tuberculosis (TB) and analyze the spatial distribution and temporal trend of TB incidence and mortality in Oyo state Nigeria from 2015–2019. The study was composed of TB cases and deaths reported by the Oyo State Health Secretariat between 2015 and 2019. The purely spatial, space-time scanning and the Getis-Ord Gi* techniques were used to analyze the spatial characteristics of TB and to identify areas at risk for its occurrence and deaths based on rates in each unit of analysis - Local Government Area (LGA), while the time series analysis technique was used to assess the characteristics of TB in Oyo state over the period studied. The standardized rates of sex and age were considered for data analysis. We analyzed 28,670 new cases of pulmonary and extra pulmonary TB as well as 1142 deaths reported in all LGAs between 2015 and 2019. Ibadan South West local government had the highest rate and risk (334 cases per 100,000), solely constituting above 10% of all cases reported in all 33 LGAs and for deaths the rate was 19.01 cases/100,000 inhabitants. Ido and Oluyole LGAs were Hot Spot regions for TB with a 99% confidence interval (CI) for TB incidence; about TB mortality, five municipalities were identified with a spatial association at 95% CI. The historical series of TB incidence rate slightly increased between the years 2015–2017, with a sharp decline from then until 2019. The temporal trend for the incidence and mortality of TB in the period under study decreased. This study contributes to knowing the epidemiological profile, the spatial and temporal distribution, and areas with higher risk of TB transmission in Oyo state. This makes it possible for policy makers to target hotspot areas for intervention and disease prevention.

Risk factors and incidence of surgical wound infection after stoma reversal: A systematic review and meta-analysis

To determine the incidence and risk factors of incisional infection following stoma reversal surgery. As of July 30, 2024, an extensive literature search was conducted on databases including PubMed, Web of Science, Cochrane, Embase, and OpenGrey. The certainty of evidence was evaluated using the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) approach. Data analysis was performed using Stata 14.0.20 articles were included, with a total sample size of 8542, including 723 patients with incision infection. The incidence of incision infection was 12%(95%CI:0.094–0.145). The results identified 5 patient-related risk factors of incisional infection, including stoma type (OR: 3.06, P = 0.015), inflammatory bowel disease (OR: 1.91, P = 0.012), Body Mass Index (BMI; OR: 1.12, P < 0.01), period from stoma creation (OR: 0.18, P = 0.012), and surgical site infection (SSI) after primary surgery (OR: 3.57, P < 0.01), and 3 surgery-related risk factors, including subcutaneous drainage (OR: 0.26, P = 0.019), suture method (OR: 4.83, P < 0.01), operation time > 60 min (OR: 4.33, P < 0.01), operation time (continuous variable, OR: 1.004, P < 0.01). Clinical staff can refer to the influential factors in this study to reduce the incidence of incision infection.

Development and validation of the Self-Awareness of Ego-Threatening Biases Questionnaire (SAETBQ)

Awareness of social biases is crucial as they impact both individual behavior and societal outcomes. Whereas previous research indicates that self-awareness of ego-nonthreatening biases enhances self-regulation, the effects of self-awareness of ego-threatening biases remain underexplored. Preliminary findings suggest that awareness of ego-threatening biases related to rumination may lead to maladaptive states. However, these findings await replication with standardized instruments. To address this gap, we conducted two studies. In Study 1 (N = 1609), we developed and validated the 12-item Self-Awareness of Ego-Threatening Biases Questionnaire (SAETBQ). Consistent with our hypotheses, self-awareness of ego-threatening biases (as measured by the SAETBQ) correlated with higher moral disengagement, lower self-diagnostic motive, and lower integrative self-knowledge, indicating a tendency towards ego deterioration, whereas self-awareness of ego-nonthreatening biases (as measured by the Metacognitive Self questionnaire) showed the opposite pattern of correlations, indicating a tendency towards beneficial self-regulation. In Study 2 (N = 681), Dark Triad traits correlated positively and Light Triad traits negatively with self-awareness of ego-threatening biases. These results underscore the complex role of self-awareness in managing cognitive biases.

In-silico trials of targeted screening for abdominal aortic aneurysms using linked healthcare data: A study protocol

Background The NHS abdominal aortic aneurysm (AAA) screening programme (NAAASP) is both clinically and economically effective. One of the main determinants of this effectiveness is disease prevalence. AAA prevalence is decreasing over time, steadily reducing the efficiency of the current NAAASP screening policy. One alternative to whole population screening is targeted screening of high-risk groups. Whether this would detect a clinically and publicly acceptable proportion of disease, and whether it would improve cost-effectiveness are unknown. The aim of this research is to estimate the clinical outcomes and cost-effectiveness of targeted AAA screening. Methods Rather than conducting an expensive and time-consuming randomized trial to directly test targeted screening, we will undertake in-silico trials of targeted AAA screening. To determine success criteria for in-silico trials, the ethics and issues around the acceptability of targeted screening will first be explored through focus groups and interviews. A qualitative evidence synthesis to identify issues associated with targeted screening will be used to establish themes and topic guides. To perform the in-silico trials, individual men’s outcomes from the NAAASP (2013–2024, ≈ 2,500,000 men, ≈ 1% with AAA) will be linked to primary care data from the Clinical Practice Research Datalink (CPRD) (20% overlap of records). Risk factors for AAA will be identified by developing a risk prediction model and used as targeted screening criteria in in-silico trials, with diagnostic accuracy as the primary outcome. A discrete event simulation model will be adopted to extrapolate the trial findings beyond the observed period. We will estimate the clinical and cost-effectiveness of targeted screening compared with the current whole population screening strategy. Data linkage will be undertaken under Health Research Authority Confidentiality Advisory Group (Section 251) approval. Linked data will be effectively anonymised. Participants in the qualitative substudy will provide informed consent for participation. Discussion We expect this project to have a direct and significant impact on NHS, UK and worldwide AAA screening policies. The study findings will be submitted for publication in peer-reviewed journals and presented at scientific meetings.

Chemometric analysis of ethoxylated polymer products using extracted MALDI-TOF-MS peak distribution features

MALDI-TOF MS (matrix-assisted laser desorption/ionization time-of-flight mass spectrometry) of ethoxylate products produces spectra with distributions of regularly spaced peaks resulting from the addition of monomer units of ethylene oxide to the oligomer. We show that overlapping peak distributions from the different ethoxylated constituents of natural raw materials can be resolved, so that features of the individual distributions (m/z at distribution maximum, intensity at the distribution maximum, width of the distribution at half height, and ratio of the distribution to the major peak distribution) can be extracted and used with statistical pattern recognition techniques to study ethoxylated products. Crucially, we weight the extracted features, so that features from a distribution with a high ratio to the main distribution are given more importance (‘ratio-scaled’). We exemplify the method by characterizing the structural variation between types of compositionally diverse Polysorbate 80, PEG castor oil and Oleth-20, and compare the chemometric analysis using our extracted features with analysis of the full spectra. We demonstrate that using ratio-scaled extracted features gives superior results to the full spectrum, both in terms of identifying subtle compositional differences that would otherwise be missed, and in interpretability. Importantly, the integrated auto-assignment of peak distributions to possible compounds allows the results to be reported in terms of the most abundant oligomers of the raw material constituents. This simplification facilitates interpretation of the results and allows the comparison of closely related products.

Spatio-temporal expansion patterns and influencing factors in China’s new local retail industry: A case study of Luckin Coffee

with the increased penetration of the Internet, artificial intelligence, and other technologies, New Retail, an emerging commercial sector in cities, has gained significant momentum. As a new local retail brand in China, Luckin Coffee has actively explored the new retail model and rapidly captured the market. Its expansion strategy and influence mechanisms are worth exploring in depth. Based on point of interest (POI) data of China’s Luckin stores (2018–2023), this study constructs a geographical indicator system and employs spatial analysis and factor detection methods to study the evolution of Luckin Coffee’s resource agglomeration patterns and its formation mechanism. The results indicate the following: (1) Luckin Coffee’s layout in China exhibits a highly concentrated and unbalanced spatial distribution pattern, showing an expansion trend from coastal regions toward inward diffusion, with most stores located in economically developed areas such as urban agglomerations and first- and second-tier cities; (2) Luckin Coffee’s spatial expansion pattern has progressed through the three phases: initially dominated by hierarchical diffusion, then by a combination of hierarchical and contact diffusion, and finally by contact diffusion, (3) the spatial differentiation in the number of Luckin Coffee stores results from the interplay of multiple factors. However, the influence of each factor on spatial differentiation varied significantly. Specifically, urban construction levels and consumption capacity within the second geographic area, along with informatization levels and human capital in the third geographic area, exhibit the greatest explanatory power regarding spatial distribution. (4) The impact of primary influencing factors on Luckin Coffee’s spatial distribution revealed spatial variability, with notable local imbalances. These imbalances are significant, and the analyzed units with relevant influences exhibit block and band aggregation characteristics. This study provides empirical evidence to supplement current research on the spatial expansion patterns and influence mechanisms of New Retail brands in China.
