Abstract The Neuronal Ceroid Lipofuscinoses (NCLs) are fatal inherited lysosomal storage diseases, with pronounced neuron loss in the central nervous system (CNS). Gastrointestinal issues are frequently reported by people with NCLs, although mechanisms underlying these symptoms are poorly understood. We recently demonstrated degeneration occurs within the enteric nervous system (ENS) in several NCLs. Given that the gut microbiome has been shown to be altered a CLN2 mouse model ( Tpp1 R207X/R207X ) and may potentially influence both CNS and ENS pathology, we investigated the long-term impact of modulating the gut microbiome in these mice. This was done by administering a VNAM antibiotic cocktail (vancomycin, neomycin, ampicillin, and metronidazole) for 1-week post-weaning, examining its effects at disease endstage. While VNAM treatment markedly altered the gut microbiome and caused significant loss of enteric neurons in wildtype mice, it did not exacerbate key pathological parameters in either bowel or brain of Tpp1 R207X/R207X mice. These included histomorphometric changes in the small intestine and neurodegeneration of enteric neurons, or CNS neuropathology. However, we did find evidence for moderate protective effects of VNAM upon enteric neurons in the ileum, and upon CNS microglia, but all other pathologies were unaltered in Tpp1 R207X/R207X mice. These findings suggest that intestinal and ENS pathology is primarily driven by TPP1-deficiency rather than changes in the gut microbiome. Indeed, these alterations to the gut microbiome may occur secondary to the impact of CLN2 disease upon the bowel.

Exploring insecticidal activity and SAR study of newly synthesized Benzo[h]quinoline-based heterocycles against Aphis craccivora Koch. and Culex pipiens L. Larvae

Abstract The key material, 2-chlorobenzo[ h ]quinoline-3-carbaldehyde condensed with various active-methylene reagents to produce some benzo[ h ]quinoline-based heterocycles like pyrazolone, imidazolone, thiazolidine, pyrimidinone, and indolinone candidates. The insecticidal activities of synthesized substrates were evaluated against cowpea aphid, Aphis craccivora , adults and common house mosquito, Culex pipiens larvae. The results of toxicological bioassays demonstrated that compounds 9 and 11 , containing 2-thioxoimidazolidine and 2-thioxothiazolidine core, exhibited high insecticidal activity against A. craccivora adults with LC 50 s of 3.54 and 3.87 µg/mL, and against C. pipiens larvae with LC 50 s of 1.61 and 1.83 µg/mL, respectively. Schiff base 7 and arylidene 5 (bearing pyrazolone scaffolds) achieved good potencies with low LC 50 values of 4.40 and 5.25 µg/mL for A. craccivora adults, and 2.16 and 2.49 µg/mL for C. pipiens larvae, respectively. On the other hand, the bulky pyrimidinetrione 16 and indolone 18 showed low toxicities. Overall, the tested substrates were more effective against C. pipiens larvae than A. craccivora adults. The current study highlighted the effect of the inserted function groups on 2-chlorobenzo[ h ]quinoline core on the compounds’ activity through the structure-activity relationship (SAR) study. This study presents promising results for the development of novel and effective insecticides to be integrated into pest control programs.

Disparity of ophthalmic surgeries in Japan

Purpose Although regional disparities in access to surgical care have been reported across medical specialties, nationwide evaluations of variation in ophthalmic surgical services remain limited. This study examined prefecture-level differences in major ophthalmic surgeries in Japan using publicly available claims data. Methods We analyzed the ninth release of the National Database of Health Insurance Claims and Specific Health Checkups of Japan (NDB Open Data) for fiscal year 2022 (April 2022–March 2023). Prefecture-level procedure counts were extracted for cataract surgery (K282), vitrectomy (K279–K281 and related codes), glaucoma surgery (relevant K-codes), and corneal transplantation. To reduce potential underestimation in non–Diagnosis Procedure Combination settings, ophthalmology-related procedures reimbursed under the Short-Stay Surgery Basic Fee 3 (A400) were identified and incorporated according to predefined mapping rules. Surgical rates per 100,000 population and per board-certified ophthalmologist were calculated. Regional inequality was quantified using population-weighted Gini coefficients. Associations between ophthalmologist density and surgical volume were assessed using Pearson’s correlation coefficients. Results Nationwide totals were 1,777,502 cataract surgeries, 154,336 vitrectomies, 80,753 glaucoma surgeries, and 2,895 corneal transplantations. Prefecture-level surgical rates per 100,000 population varied 1.8-fold for cataract surgery, 3.3-fold for vitrectomy, and 7.8-fold for glaucoma surgery. Several prefectures reported no corneal transplantation cases. Population-weighted Gini coefficients were 0.064 for cataract surgery, 0.125 for vitrectomy, 0.190 for glaucoma surgery, and 0.351 for corneal transplantation. Similar patterns were observed after adjusting for age structure, indicating that regional disparities were not solely explained by demographic differences. Ophthalmologist density varied 2.5-fold across prefectures and was positively correlated with surgical volume for cataract surgery and vitrectomy, but not for glaucoma surgery. Conclusions Using nationwide claims data, we identified substantial regional variation in major ophthalmic surgical procedures in Japan, with greater inequality observed for more specialized surgeries. These findings provide a population-level description of ophthalmic surgical distribution and may inform future evaluations of healthcare resource allocation.

Who belongs? Co-creating an assessment to measure belonging in a community space

It is inherent to human nature to want to belong, and a strong sense of belonging has been closely correlated with multiple metrics of wellbeing on an individual and community level. Ensuring improvement in sense of belonging requires accurate measurement of belonging within the community of interest, an effort that has previously been constrained to primarily education and employment spaces and often lacking community voice. In this paper, we describe a novel approach to survey development, wherein we combine community voice with existent validated survey tools to develop a measure of a construct – sense of belonging in this instance – within a timeframe that better fits community desired pace for progress and change. In partnership with the YMCA of Greater Cincinnati, we worked within a Community Based Participatory Research (CBPR) approach to measure sense of belonging using a combination of literature review, extraction and qualitative research methods. External member checking and a Cronbach alpha analysis illustrate success in measuring the construct of interest: sense of belonging within the context of YMCA membership.

A need or a luxury? Parents’ attitudes towards international schools from a linguistic perspective

English is the language of instruction at international schools in Saudi Arabia, which have traditionally been attended by the children of expatriate professionals living in the country. In recent years, Saudi parents have begun to enrol their children in international schools, which is a novel phenomenon in the Saudi context. This study assessed parents’ attitudes towards international schools from a linguistic perspective as well as their attitudes towards the potential impact on their children’s first language – Arabic. Using a mixed-methods research design, both quantitative and qualitative data were collected from 76 parents using a 30-item questionnaire with a combination of open-ended and close-ended question. The findings indicate that Saudi parents choose international schools for a variety of reasons, including strong reputations in science education (72.4% of the sample), high quality curricula (80%), and the intensive English input required in various university specialisations and the labour market since Saudi Arabia (85%). Other reasons and concerns were also reported by some parents in the qualitative data. Saudi families returning from time spent in English-speaking countries choose international programmes for their monolingual children, either as a bridge before enrolment in public schools or as a long-term option. Despite the increasing popularity of international schools, Saudi parents nevertheless report concerns about the influence of international schools on children’s first language acquisition.
