The Retinal Light Response Is Modulated by an mGluR5-Mediated Retrograde Signal from ON-Bipolar Cells to Photoreceptors
The ON visual pathway is initiated by the deactivation of mGluR6, coupled to the opening of TRPM1 channels in retinal ON-bipolar cell dendrites. Here, we show that a second metabotropic glutamate receptor, mGluR5, is localized with TRPM1 and mGluR6 in the dendrites of ON-bipolar cells. To examine the function of mGluR5, we performed electroretinogram (ERG) recordings in mice of either sex and found that the amplitude of the b-wave, which is primarily a measure of ON-bipolar cell light-driven activity, is reduced in mGluR5 knock-out mice compared with wild type. In the mGluR5 −/− retina, we observed weaker mGluR6 immunofluorescence in the dendritic tips of ON-bipolar cells that could explain the smaller ERG b-wave. To observe the effect of mGluR5 without perturbing mGluR6 expression, wild-type mice were injected with MTEP, an allosteric antagonist of mGluR5. MTEP increased the amplitude of the b-wave in response to dim stimuli and caused an inflection in the intensity–response plot for flashes in the mesopic range. In the brain, postsynaptic mGluR5 regulates presynaptic glutamate release via endocanabinoid-mediated retrograde signaling. Therefore, we tested the effect of the CB1 receptor antagonist, SR1417A, on the ERG and found that the b-wave was affected as by MTEP, including an inflection in the intensity–response. We further showed that the CB1 receptor agonist, ACEA, reversed the effects of MTEP. Together, our results indicate that mGluR5 plays a role in gain-control at the photoreceptor to ON-bipolar cell synapses, likely via an endocannabinoid-mediated retrograde feedback.

Bilateral muscle activation in postparalytic facial synkinesis: a cross-sectional high-resolution surface electromyography study
Abstract Facial synkinesis (FS) is a facial muscle coordination disorder following facial paralysis. We hypothesized that FS involves the entire network of facial muscles on both sides of the face. Bilateral facial high-resolution surface electromyography (HR-sEMG), using a total of 58 electrodes, was performed during 11 standardized facial movements in 36 healthy adult participants (53% female, 18–67 years) and 36 patients with FS (81% female, 24–70 years). Differences between the synkinetic and contralateral sides of patients and healthy controls were evaluated using linear mixed-effects models. Each movement task elicited a complex activation pattern involving nearly all measured facial muscles. In healthy controls, HR-sEMG activity was highest in the muscles primarily responsible for generating the intended voluntary movement. Notably, in patients with FS, muscles not normally expected to be strongly involved in a given movement showed markedly higher activity on the synkinetic side, and often also on the contralateral side, compared with healthy controls. Thus, facial movements in FS evoke a complex pattern of activity that only partially serves the intended movement and is further characterized by involuntary activation in muscles outside the primary movement region and on the contralateral side.

Prevalence and associated factors of neonatal hypothermia in the neonatal intensive care unit at St. Peter’s specialized Hospital, Ethiopia
Abstract Neonatal hypothermia, defined as a body temperature below 36.5 °C, is a global issue that significantly increases neonatal morbidity and mortality, particularly in low- and middle-income nations. Despite global guidelines, the prevalence remains high in many resource-limited settings. This study aimed to assess the prevalence and associated factors of hypothermia among neonates admitted to the intensive care unit of St. Peter’s Specialized Hospital. An institution-based cross-sectional study was conducted from July to September 2024 among 222 neonates admitted to the intensive care unit of St Peter’s Specialized Hospital. The data were collected using a semi-structured questionnaire and chart review. Binary and multivariate logistic regression analyses were used to identify factors associated with hypothermia at a significance level of p < 0.05 using Statistical Package for Social Sciences (SPSS) version 25.0 software. Prevalence of neonatal hypothermia was 66.7%. Neonates from families earning ≤ $100 (AOR = 2.60, 95% CI: 1.25, 5.42), aged ≤ 24 h (AOR = 2.85, 95% CI: 1.85, 5.05), with a gestational age < 37 weeks (AOR = 4.50, 95% CI: 2.40, 6.50), with a lack of thermal care (AOR = 2.75, 95% CI: 1.50, 4.40), with improper wrapping at admission (AOR = 5.50, 95% CI: 3.60, 9.80), and with inborn delivery (AOR = 2.75, 95% CI: 1.45, 5.00) increased the odds of hypothermia. However, single pregnancy (AOR = 0.36, 95% CI: 0.15, 0.88) decreases the odds of hypothermia. Neonatal hypothermia remains highly prevalent among neonates admitted to the intensive care unit of St. Peter’s Specialized Hospital. Significant risk factors were low income, neonates aged ≤ 24 h, gestational age < 37 weeks, lack of thermal care, improper wrapping at admission, and inborn delivery. While a single pregnancy is a proactive factor. Adherence to thermal care practices guidelines, particularly during the first hours of life, is crucial. Healthcare facilities should enforce policies, provide regular training, and evaluate the effectiveness of national and World Health Organization guidelines.
