Attitudes and practices of healthcare professionals in a comprehensive tertiary hospital regarding traditional Chinese medicine for the treatment of influenza
Abstract This study aimed to evaluate the attitudes and practices of healthcare professionals at a comprehensive tertiary hospital regarding the use of Traditional Chinese Medicine (TCM) in influenza treatment. A cross-sectional survey was conducted at Quzhou Affiliated Hospital of Wenzhou Medical University between June 5 and July 14, 2024. Data were collected using a validated self-designed questionnaire assessing healthcare professionals’ attitudes and practices. Analysis of 606 questionnaires (95.43% response rate) revealed 70.79% doctors (59.08% female); 79.21% had TCM influenza experience. Mean attitude score (50.69 ± 6.73) indicated favorable TCM perceptions, but practice scores (28.47 ± 5.43) showed moderate engagement. Attitude and practice scores correlated significantly (*r* = 0.5252, P < 0.001). Educational events ( P < 0.01), family TCM use ( P < 0.001), and TCM-employed relatives ( P < 0.05) mediated attitudes and practices. Although healthcare professionals demonstrated generally positive attitudes towards TCM, their actual clinical practice remained limited. To promote the integration of TCM into influenza care, targeted training and educational programs are recommended.

Abstract Structured nucleic acids play key roles in a variety of molecular mechanisms and have numerous applications in medicine and biotechnology. Stem-loops with specific tetraloop sequences, such as UNCG and GNRA, are among the most well-studied recurrent building blocks of structured RNAs, known to adopt very stable structures. The GNRA tetraloop motif frequently engages in long-range interactions with specific RNA receptors and can also be recognized by proteins. While several examples of GNRA-like conformations have been reported in non-tetraloop strands, no systematic survey has been conducted to explore the landscape of these variants. In this work, we report a comprehensive survey of GNRA tetraloop-like motifs within known 3D structures of nucleic acids, based solely on isostericity and without imposing restraints on sequence, loop type, backbone topology, or interactions involved. We identified twelve recurrent backbone topology variants of the motif, including all five possible two-strand variants and six three-strand variants. Among the pentaloop variants, we observed four different locations of the looped-out residues, with three of them directly interacting with proteins. These GNRA-like motifs highlight the versatility of favorable RNA 3D conformations to fit in diverse backbone contexts and modulate intermolecular interactions, which can be leveraged in RNA motif design.
Precipitation timing mediates life-stage and population-level associations with climate for an indicator species
Abstract Global climate change is contributing to declines in biodiversity, although changes vary across geographic regions and species. The iconic greater sage-grouse ( Centrocercus urophasianus; sage-grouse) is central to conservation within the North American sagebrush ecosystem, yet its vulnerability to climate effects remains poorly understood. We used hierarchical models to explore weather and climatic influences on sage-grouse across nearly four decades, focusing first on pattern (population change), then process (demographic life stage). We quantified eight indicators of temperature and/or moisture conditions across time lags of 0–1.5 years leading up to current year breeding activities. Sage-grouse population growth tended to increase following above-average growing season precipitation. Precipitation or drought timing influenced whether process effects were positive or negative. More summer and/or fall moisture corresponded with increases in subsequent year brood and adult survival, while concurrent seasonal drought led to reductions in nest survival. Positive concurrent spring drought associations with adult and brood survival were largely outweighed by positive prior year moisture effects. Density-dependence and indirect effects of precipitation through vegetation and forage resource pathways likely contributed to nuanced responses across life stages. Our research improves mechanistic understanding of this indicator species’ sensitivity to climatic factors, while reinforcing the importance of large-scale conservation initiatives for sagebrush ecosystems.

Study of epinephrine-induced cardiovascular adverse events in patients with anaphylaxis using two spontaneously reported adverse event databases from 2004 to 2024
Abstract Anaphylaxis is a life-threatening hypersensitivity reaction requiring prompt administration of epinephrine. Despite its life-saving role, hesitancy in epinephrine use persists, largely due to concerns about cardiovascular adverse events (AEs). This study aimed to characterize potentially epinephrine-induced cardiovascular AEs in anaphylaxis using data from two major pharmacovigilance databases. A retrospective analysis was conducted using individual case safety reports (ICSRs) from the US FDA Adverse Event Reporting System (FAERS) and the European EudraVigilance database from 2004 to 2024. ICSRs were included if epinephrine was a suspect drug and the indication involved anaphylaxis or related terms. Reports were screened for cardiovascular AEs and assessed for causality by two independent reviewers. Descriptive statistics were used to summarize findings. A total of 802 and 517 ICSRs were analyzed from FAERS and EudraVigilance, respectively. Cardiovascular AEs were reported in 142 and 134 ICSRs, respectively, with epinephrine as the sole suspect in most cases. Female patients were more frequently represented. Stress cardiomyopathy, myocardial ischemia, tachycardia, and acute myocardial infarction were the most common cardiovascular AEs. Intravenous administration was frequently associated with serious outcomes. Causality assessment indicated that a majority of cardiovascular AEs were likely related to epinephrine. Cardiovascular AEs, particularly stress cardiomyopathy, are associated with epinephrine use in anaphylaxis, especially when administered intravenously. While these findings support cautious use, they also highlight the importance of adhering to recommended intramuscular administration to minimize preventable risks. Clinician awareness and adherence to guidelines remain critical for safe and effective anaphylaxis management, prioritizing cautious use over hesitancy.
