Lessons learned from operationalizing the integration of nutrition-specific and nutrition-sensitive interventions in rural Ethiopia

Background Undernutrition reduction requires coordinated efforts across sectors to address its causes. A multisectoral approach is important in diagnosing the problem and identifying solutions that would be implemented across different sectors. The study aimed to explore the experience of health and agriculture extension workers in integrating nutrition-specific and nutrition-sensitive interventions provided to households with children under two years of age at community level. Methods A qualitative study was conducted in agrarian areas of Ethiopia following the completion of a multi-sectoral program that integrated health and agriculture interventions, in 2021. The program’s goal was to reduce stunting and improve the dietary diversity of young children. In total, 28 key informant interviews were conducted with health- and agriculture-extension workers and mothers. A framework analysis approach was applied to manage and analyze data using NVivo version 12 software. Results The study showed that joint health and agriculture interventions improved community knowledge on childcare and agricultural practices. The practice of farm gardening and cooking demonstrations were improved after the implementation of the program. Because of service integration, extension workers perceived an improved father’s role in supporting mothers in childcaring and feeding nutritious diets to children and decreased severe cases of undernutrition. Integration of health and agriculture sectors for nutrition intervention was challenged by the high workload on extension workers, poor supervision and leadership commitment, lack of appropriate agricultural inputs, and absence of clarity on sector-specific roles. In some areas nutrition services are not owned by the health and agriculture sectors, and it was overlooked. Conclusion Integrating community-level platforms were key entry points to address undernutrition and promote key agriculture and health interventions. The joint implementation of health and agriculture services were effective in the reduction of wasting and improved the role of family members in supporting mothers. However, Integration of sectors were facing challenges in creating shared vision to improve nutrition status of children and women, distributing workload and equal commitment among sectors.

Repeated exposure decreases aesthetic chills likelihood but increases intensity

Aesthetic chills are a peak emotional response to stimuli such as music, films, or speech characterized by shivers and goosebumps and activation of dopaminergic circuits. Despite growing scientific interest in this phenomenon, repeated exposure to chills stimuli has not been studied yet, due to the absence of a validated database. This study leverages a recent gold standard in chills stimuli to investigate the impact of repeated exposure on the frequency and intensity of aesthetic chills. Participants (n = 58) were randomly exposed to 6 chill-evoking stimuli pre-validated on the population of interest, in a counterbalanced order. Our findings revealed a significant decrease in the likelihood of experiencing chills with repeated exposure, suggesting habituation to chills itself or potential fatigue in response to aesthetic stimuli. However, we observed an increase in the intensity and duration of chills over successive exposures among those who did experience chills. The study also identified distinct demographic and psychophysiological response patterns across different participant groups, indicating variability in chill responses. These results provide insights into the dynamic nature of aesthetic experiences and their underlying neural mechanisms, with implications for understanding emotional and reward processing in psychophysiology.

Association of tobacco use with depressive symptoms in adults: Considerations of symptom severity, symptom clusters, and sex

Objective We aim to assess the association between depressive symptoms, depressive symptom severity and symptom clusters with tobacco use. We will also evaluate sex differences in these associations. Method This cross-sectional study used data from the National Health and Nutrition Examination Survey (2005-2018). Depressive symptoms were assessed using the Patient Health Questionnaire-9. Tobacco use was categorized into four groups: cigarette use, smoked tobacco products (pipes and cigars), smokeless tobacco products (chewing tobacco and snuff), and non-tobacco use (reference group). Results This study included 33,509 participants. Cigarette use was associated with a 0.83-unit increase in total PHQ-9 scores (95% CI =  [0.63, 1.04]), and 1.73 times higher odds of reporting depressive symptoms (95% CI =  [1.48, 2.02]) compared to non-tobacco use. However, the use of smoked and smokeless tobacco was not associated with depressive symptoms. In females, cigarette use showed a stronger association with total PHQ-9 scores (aCoef =  1.23, 95% CI =  [0.92, 1.55]) than in males (aCoef =  0.45, 95% CI =  [0.21, 0.69]). Additionally, female smoked tobacco users showed positive associations with both PHQ-9 scores and the presence of depressive symptoms, but this relationship was not observed in males. Furthermore, subgroup analysis revealed associations between cigarette use and cognitive-affective and somatic symptom clusters, as well as a relationship between the logarithm of total cigarette consumption and depressive symptoms. Conclusion Cigarette use was associated with higher odds of depressive symptoms with females having a stronger association. Further studies are needed to replicate these findings and examine the underlying mechanisms.

Protocol of the PROMOTE study: characterization of the microbiome, the immune response, and one-carbon metabolism in preconceptional and pregnant women with and without obesity (an observational subcohort of the Rotterdam Periconception cohort)

Introduction Preconceptional and maternal obesity are well-known risk factors for pregnancy and fetal complications including gestational diabetes, hypertensive disorders, and macrosomia. Maternal obesity is associated with offspring obesity and increased healthcare costs. To disrupt the cycle of obesity, we aim to investigate the impact of the composition of the maternal microbiota (bacteria and viruses) throughout preconception and pregnancy and the associations with the immune responses and one-carbon metabolism (1-CM) as an underlying mechanism in the pathophysiology of increased adverse pregnancy outcomes in maternal obesity. Methods and analysis The PROMOTE study is a subcohort of the Rotterdam Periconceptional Cohort, a hospital-based observational cohort study. We will include 70 women per BMI group: ≥ 30 kg/m2 or 18.5-25 kg/m2, at different time points in each group: 10 preconceptional, 50 in the first trimester (with longitudinal follow-up during pregnancy, delivery and postpartum) and 10 in the third trimester of pregnancy. Which makes a total of 140 inclusions. Vaginal and rectal bacteriome, virome, and blood samples are collected. In the third trimester inclusions, only faecal samples are collected. Microbiota samples will be analysed using 16S rRNA sequencing. Bacteriome and virome profiles are compared between the BMI subgroups, associations with general immune responses and 1-CM markers will be shown. Trial registration ClinicalTrials.gov (NCT05754645)

Integrating virtual reality, electroencephalography, and transcranial magnetic stimulation to study the neural correlates of awe experiences: The SUBRAIN protocol

Introduction Awe is a complex emotion unveiling a positive and mixed nature, which resembles the Romantic feeling of the Sublime. It has increasingly become the object of scientific investigation in the last twenty years. However, its underlying brain mechanisms are still unclear. To fully capture its nature in the lab, researchers have increasingly relied on virtual reality (VR) as an emotion-elicitation method, which can resemble even complex phenomena in a limited space. In this work, a multidisciplinary team proposed a novel experimental protocol integrating VR, electroencephalography (EEG), and transcranial magnetic stimulation (TMS) to investigate the brain mechanisms of this emotion. Methods A group of bioengineers, psychologists, psychiatrists, and philosophers designed the SUBRAIN study, a single-center, one-arm, non-randomized interventional study to explore the neural processes underlying awe experiences. The study is ongoing and is expected to enroll fifty adults between 20 and 40 years of age. Currently, more than 40 individuals have been enrolled. The experimental protocol includes different steps: (i) screening, (ii) enrollment, (iii) pre-experimental assessment, (iv) VR experimental assessment, and (v) post-experimental debriefing. The brain’s electrical activity is recorded using the EEG while participants navigated three immersive awe-inducing VR environments and a neutral one. At the same time, the cortical excitability and connectivity is investigated by performing a TMS-EEG session right after each VR navigation. Along with cerebral signals, self-reported questionnaires were used to assess the VR-induced changes in the emotional state of the subjects. This data is then analyzed to delve into the cerebral mechanisms of awe. Discussion This study protocol is the first one that tries to fully understand the neural bases of awe by eliciting and studying this phenomenon in VR. The pairing of awe-inducing VR experiences and questionnaires investigating participants’ affect and emotions, with non-invasive neural techniques, can provide a novel and extensive knowledge on this complex phenomenon. The protocol can inform on the combination of different instruments showing a reproducible and reliable setting for the investigation of induced complex emotions.

Identification of mechanistic CKD biomarkers in a rat SNx kidney fibrosis model by transcriptomics and proteomics detectable in biofluids

Abstract The rat sub-total nephrectomy (SNx) is a functional model of general chronic kidney disease (CKD) where the main pathological driver is glomerular hypertension representative of several subtypes of CKD. Comprehensive transcriptomics and proteomics analyses on the SNx rats were performed to identify biomarkers in plasma or urine that correlate with kidney disease and functional kidney loss. Kidneys were subjected to collagen I and III staining for fibrosis scoring, SWATH-MS proteomics and bulk RNA-sequencing transcriptomics, with SWATH-MS also performed on plasma and urine. Differential expression analysis demonstrated significant dysregulation of genes and proteins involved in fibrosis, metabolism, and immune response in the SNx rats compared to controls. Gene ontology analysis of the intersecting genes and proteins from both studies demonstrated common biology between animal cohorts that reached the predefined kidney disease thresholds (serum creatinine > two-fold or proteinuria > three-fold increase over sham-operated). Thirteen significantly differential molecules were detected with consistent directional changes in both omics datasets. These molecules were detected independently in kidney (both RNA and protein) and urine (protein only), but not in plasma. Bioinformatics analysis enabled the identification of mechanistic CKD biomarkers including lumican and collagen alpha-1(III) chain, whose co-expression has previously been both implicated in fibrosis and detected in urine in CKD patients.

Quantitative trait loci mapping for salt tolerance-related traits during the germination stage of wheat

Soil salinization is a type of abiotic stress that affects the growth and development of wheat. To explore the QTLs related to salt tolerance during the germination stage of wheat and to reveal the mechanisms of salt tolerance, this study subjected 196 wheat varieties (lines) from North China, East China, and Central China to salt stress during the germination stage. Principal component analysis was employed for a comprehensive evaluation of salt tolerance. Based on the comprehensive evaluation D value, the salt tolerance of the research materials was classified into five levels, from which 64 materials exhibiting salt tolerance or higher were selected. Further, a genome-wide association analysis was conducted on the phenotypic traits and D values of wheat under different treatments during the germination stage using sequencing data from a 16Kb SNP chip. A total of 108 QTLs significantly associated with salt tolerance during the germination stage were identified, distributed across 15 chromosomes, excluding 1A, 1D, 4A, 5B, 6B, and 7B. Individual significant SNPs could explain 8.03% to 22.62% of the phenotypic variation. Additionally, 11 candidate genes potentially related to the salt response in wheat were predicted. This study provides a theoretical basis for the cloning of salt tolerance-related genes and the breeding of salt-tolerant wheat varieties.

Spatial multilevel analysis of individual, household, and community factors associated with COVID-19 vaccine hesitancy in the Dominican Republic

Abstract Vaccine hesitancy or refusal poses a significant public health challenge resulting in the resurgence of preventable diseases and undermining the effectiveness of national and global health initiatives. This study investigates multilevel determinants of COVID-19 vaccine hesitancy in the Dominican Republic (DR) shortly after the launch of the national COVID-19 vaccination campaign in February 2021. Participants aged 18 years and older were enrolled through a national multistage cluster survey conducted from June-October 2021. The Health Belief Model guided the selection of potential factors contributing to vaccine hesitancy. Hierarchical mixed-effect logistic regression models were used to examine individual, household, and community factors associated with vaccine hesitancy. COVID-19 vaccine hesitancy was observed in 12.6% (95% CI: 11.7–13.5%) of participants (n = 5,566), with spatial variations at the cluster level. Individual factors associated with lower odds of vaccine hesitancy included older age, higher education levels, mulatto ethnicity, and perceiving vaccination as crucial for health. In contrast, factors significantly associated with hesitancy included being born in the DR and concerns about COVID-19 vaccine side effects. For factors at the household level, differential trust in health information sources significantly influenced vaccine hesitancy, with certain sources correlating with increased hesitancy and others with reduced vaccine hesitancy. Better access to healthcare, as indicated by a higher number of hospitals per population, was paradoxically associated with increased vaccine hesitancy. Future strategies to reduce vaccine hesitancy in the DR might consider these multifaceted factors.
