Digital biomarkers for interstitial glucose prediction in healthy individuals using wearables and machine learning

Abstract A personalized low-glycemic diet, maintaining stable blood glucose levels, aids in weight reduction and managing (pre-)diabetes and migraines in individuals. However, invasiveness, high cost, and limited lifecycle of continuous glucose monitoring (CGM) devices restrict their widespread use. To address these issues, we investigated machine learning (ML) approaches for glucose monitoring using data from non-invasive wearables. Our study comprised two phases involving healthy participants: The main study included two experimental sessions lasting 7–8 h with two standardized test meals, totaling over 1550 interstitial glucose (IG) measurements with CGM, and high-frequency multimodal data collected by two different non-invasive sensor devices. The follow-up study involved more than 14,400 IG measurements. Using ML approaches, correlations between glycemic measures and sensor data were assessed to estimate the feasibility of accurately predicting personalized IG alterations in real-time. An ensemble feature selection-based light gradient boosting machine (LightGBM) algorithm, omitting the need for food logs, was developed. This algorithm achieved a root mean squared error (RMSE) of 18.49 ± 0.1 mg/dL and a mean absolute percentage error (MAPE) of 15.58 ± 0.09%, demonstrating the feasibility of non-invasive glucose monitoring with high accuracy, which paves the way for novel approaches in the objective prevention of diet-related diseases.

Abstract Low-grade inflammation associated with the aging process exerts functional and morphological effects on multiple organs. The kidney is an organ that is severely affected by the aging process. Our study aimed to determine the relationship between histopathological and immunohistochemical changes and age-related amyloid accumulation in rat kidney tissue. Wistar albino rats were divided into two groups: young adult and elder (n = 8, 24 and 104 weeks old, respectively). H&E, PAS, and Congo red staining and immunohistochemical staining for TNF-α, IL-6, occludin, GRP78, and mitogen-activated protein kinase (MAPK) were performed on the kidney tissues of the subjects. A significant difference was found between the groups in terms of glomerulus diameter/Bowman’s capsule diameter, percentage of sclerotic glomeruli, epithelial desquamation, loss of brush border, casts, tubular dilatation, cellular debris, and the presence of apoptotic cells. TNF-α, IL-6, GRP78, and MAPK protein levels were shown to increase significantly in aged rat kidneys, whereas occludin levels decreased significantly. No amyloid accumulation was found in the kidney, heart, skin, and small intestine tissues of either group. The results of our study suggest that the proinflammatory microenvironment required for AA amyloid deposition in systemic amyloidosis induced by low-grade inflammation in old age cannot be generated by the action of NF-κB, MAPK, and MAPK-activated JNK and AP-1 transcription factors alone.

Abstract Campylobacteriosis is a significant zoonosis with major public health implications. This study aimed to investigate the prevalence, molecular characteristics, and antimicrobial resistance of Campylobacter spp. in animal (rectal swabs and milk), environmental (fecal, water, and wall swabs), and human samples (stool and hand swabs) in New Valley Governorate, Egypt. Among 573 samples analyzed, Campylobacter spp. prevalence was highest in rectal swabs (32.9%) and human stool samples (74.2%), with lower rates in fecal samples (25.9%), milk (25.7%), wall swabs (13.1%), and water (10%). All isolates showed 100% resistance to clindamycin, while being completely sensitive to imipenem and amikacin. Multidrug resistance was found in 90.1% of the isolates, and 16S rRNA was detected in 90% of randomly selected Campylobacter spp. The hipO, cadF, and ceuE genes were detected in 77.8%, 33.3%, and 22.2% of the isolates, respectively. Phylogenetic analysis of the 16S rRNA gene showed significant congruence either between the tested isolates and each other or with other isolates in the gene bank, confirming the zoonotic transmission of multi-drug-resistant Campylobacter spp. This highlights the urgent need for improved biosecurity on farms, better food handling practices, and heightened public health awareness to mitigate the risk of Campylobacteriosis.
