Gut microbiota dysbiosis, sarcopenia, osteoporosis and osteosarcopenia in older people: A systematic review protocol

Introduction Sarcopenia and Osteoporosis are two prevalent conditions in the older population and are defined by low strength, muscle quality/volume and low Bone Mineral Density, respectively. When there is a concomitant presence of both, there is a novel musculoskeletal condition called Osteosarcopenia. These conditions adversely affect quality of life and elevate the risk of fractures, disability, and mortality among older individuals. Dysbiosis of the gut microbiota is the impairment of the mutualistic relationship between microorganisms, metabolic products and the host’s immune system. Gut microbiota dysbiosis could be intricately linked to sarcopenia and osteoporosis, shedding light on the complex microbiota-gut-bone-muscle axis. Furthermore, the intestinal microbiota experiences a notable decline in beneficial microorganisms as part of the aging process. The relationship between dysbiosis of the intestinal microbiota in older people and sarcopenia, osteoporosis or osteosarcopenia is still unclear. This review protocol aims to systematically review the literature and compile evidence on the influence of gut microbiota dysbiosis on musculoskeletal function in older people with sarcopenia, osteoporosis or osteosarcopenia. Methods/Analysis This systematic review will analyze observational studies that have investigated the relationship between the effects of gut microbiota dysbiosis and sarcopenia, osteoporosis and osteosarcopenia in older people aged 65 and over. Studies will be retrieved from PubMed/MEDLINE, EMBASE, Scopus, Web of Science and the Cochrane Library. Outcome measures will include body composition for diagnosing osteoporosis and screening for sarcopenia/osteosarcopenia by any criteria. Data synthesis will involve quantitative analysis using summary measures. If sufficient studies, homogeneity and heterogeneity analysis will be performed to conduct Meta-analysis and pooled OR, RR and HR measures will be provided.

Block selection in multiblock partial least squares for modeling genotype-phenotype relations in Saccharomyces

In data-based modeling, correlations between explanatory variables often lead to the formation of distinct gene blocks. This study focuses on identifying influential gene blocks and key variables within these blocks, with a particular application in mind: genotype-phenotype mapping in Saccharomyces. To overcome the challenges of a limited sample size, we use partial least squares (PLS). These gene blocks, which consist of combinations of genes, play a critical role in explaining phenotypic variations. Using partial least squares with multiple blocks, we propose a novel approach, weighted block importance on projection in partial least squares (BwIP-mbPLS), to identify influential gene blocks. Variable importance on projection is used to select significant genes within these blocks. Our study models copper chloride at 0.375mM and melibiose at 2% efficiency and rate in Saccharomyces cerevisiae yeast. Analysis based on silhouette index and total distance within clusters using k-means shows the classification of 5629 genes into 18 gene blocks. Remarkably, BwIP-mbPLS identifies 4 gene blocks on average and significantly improves the prediction of efficiency-based phenotypes. In contrast, traditional block importance in partial least squares projection identifies 6 gene blocks on average and shows comparable or better performance than BIP-mbPLS for rate-based phenotypes. Remarkably, most gene blocks contain fewer than 10 influential genes. Both proposed variants consistently outperform conventional approaches such as partial least squares and multi-block partial least squares in predicting phenotypes. These results highlight the potential of our methods for advancing data-based modeling and genotype-phenotype mapping.

Influence of pre-processing criteria on analysis of accelerometry-based physical activity

Background Accelerometers are widely adopted for physical activity (PA) measurement. Accelerometry data require pre-processing before entering formal statistical analyses. Many pre-processing criteria may influence PA outcomes and the processed sample, impacting results in subsequent statistical analyses. Aim To study the implications of pre-processing criteria for accelerometer data on outputs of interest in physical activity studies. Methods We used the ActiGraph hip-worn accelerometry data from 538 adult Latino participants. We studied four most important domains of pre-processing criteria (wear-time, minimum wear-time, intensity level, and modified bouts). We examined the true sample size in pre-processed data, the moderate-to-vigorous physical activity (MVPA) outcome, and regression coefficients of age and gender predicting MVPA. Results Many pre-processing criteria have minimum impact to the output of interest. However, requirements for minimum wear-time can have high influence on subsequent analyses for MVPA. High requirements for wear-time (e.g., minimum of 5 days with more than 12 hours of wear-time per day) lead to weakened statistical efficiency in estimating the relationship between potential predictors and the MVPA outcome. Intensity levels using vector magnitude triaxial counts yielded drastically different results than those using conventional vertical axis counts. Conclusion Moderate changes in minimum wear-time can yield notably different output data and subsequently influence analyses assessing the impacts of interventions on MVPA behaviors. Processed data using vector magnitude and conventional vertical axis counts are not directly comparable. Sensitivity analyses using alternative pre-processing scenarios are highly recommended to verify the robustness of analyses for accelerometry data.

Spatial analysis of socioeconomic data and its relationship with illicit crops in Nariño-Colombia

The Sustainable Development Goals (SDGs) aim to eradicate poverty and promote sustainable development; however, socioeconomic disparities persist globally, particularly in Colombia. With a Gini index of 0.556 in 2022, Colombia ranks among the most unequal countries in Latin America, with its southwest region of Nariño facing severe socioeconomic challenges. Concurrently, Nariño registers the highest levels of coca cultivation in Colombia, accounting for 65% of national cocaine production, reflecting the region’s precarious conditions. This study investigates the extent to which the spatial distribution of socioeconomic factors explains coca cultivation patterns in Nariño. Grounded in conflict economics, social capital, and social marginalization theories, the research constructs composite indices representing education, health, public services, economic conditions, and vulnerability. Using spatial analysis, it identifies areas with heightened poverty and vulnerability and examines their relationship with illicit crops. The findings highlight spatial non-stationarity in the factors influencing coca cultivation, offering region-specific insights and policy recommendations to combat illicit crops and foster sustainable development. These results provide a foundation for targeted interventions and contribute to broader strategies addressing inequality and illegal economies in Colombia.

Understanding displacement of onboard contingents in Navy amphibious ships

The Naval Ship Code (NSC) was enacted in 2009 to standardize regulations for NATO member naval forces, and a study commissioned by the Spanish Navy General Staff (EMA) aimed to identify the factors that influence onboard personnel’s ability to move during an evacuation process. This study validated the soundness of the safety protocols implemented on navy vessels and highlighted the impact of certain characteristics of the embarked military contingent, such as body mass index, age, and seniority. It also found that such characteristics could act as distinctive factors among the embarked contingents in the evacuation of a military vessel. The study quantified the effect of these intervening characteristics, confirming the need for different displacement models for each of the study contingents to improve ship evacuation maneuvers. The findings of this study provide insights into the behavior of different embarked contingents during the evacuation process and can inform the development of more effective safety protocols for military naval operations. The starting hypothesis is that certain characteristics of the embarked military contingent have a decisive influence on their displacement capacity during the evacuation process. This hypothesis has been expanded in the sense that these same characteristics can act as differentiating elements among the embarked contingents evacuating a military vessel. It is possible to quantify the influence of these characteristics and implement a displacement model applicable in escape, evacuation, and rescue processes. Thus, the specific characteristics of a study contingent will be reflected in its displacement model. In this article we find that while members of the landing force (LF) show greater displacement capacity through a longitudinal corridor (around 10%), their ability to overcome other passage elements present on the study vessel is reduced (around 30%) compared to members of the vessel’s own crew.

The forces behind social unrest: Evidence from the Covid-19 pandemic

The unprecedented consequences of the Covid-19 pandemic have raised concerns about the erosion of social cohesion and intensified social unrest, but evidence for such a link and the underlying channels is still lacking. We use a unique combination of nationally representative survey data, event data on social unrest, and data on Covid-19 fatalities and unemployment at a weekly resolution to investigate the forces behind social cohesion and unrest in the context of the strains on public health and the economy due to the pandemic in the USA. The results show that pandemic-related unemployment and Covid-19 fatalities intensified negative emotional stress and led to a deterioration of economic confidence among individuals. The prevalence of negative emotional stress, particularly in economically strained and politically polarized environments, was, in turn, associated with intensified social unrest as measured by political protests. No such link is found for economic perceptions.

Living alone predicts non-home discharge post elective hip arthroplasty: A matched-pair cohort study

The impact of home support and interaction with family members on recovery and perioperative outcomes remains unclear. We determined whether living alone was predictive of discharge disposition following total hip arthroplasty (THA). Data were from American College of Surgeons National Surgical Quality Improvement Program participating hospitals in 2021. The primary endpoint was discharging disposition. A total of 1716 patients living alone and 3961 with others at home were identified. The 1:1 propensity-matched cohort included 3248 total patients (1624 in each group). On univariate analysis, living alone was associated with non-home discharge (22.0% [358/1624] vs. 10.5% [170/1623]; odds ratio [OR], 2.42 [95% CI, 1.98 to 2.94]; P < .001), need for services in those returning home (63.1% [799/1266] vs. 57.7% [839/1453]; OR, 1.25 [95% CI, 1.07 to 1.46]; P = .004), and increased length of hospital stay (2.05 vs. 1.72 days; mean difference, 0.34 [95% CI, 0.18 to 0.49]; P < .001). On multivariable analysis, living alone remained an independent predictor of non-home discharge (adjusted odds ratio, 2.84 [95% CI, 2.30 to 3.54]; c = 0.734). Thus, compared to propensity-matched THA patients with others at home, those living alone experience a much greater rate of non-home discharge and need for support.

Formation of individual stripes in a mixed-dimensional cold-atom Fermi–Hubbard system

AbstractThe relation between d-wave superconductivity and stripes is fundamental to the understanding of ordered phases in high-temperature cuprate superconductors1–6. These phases can be strongly influenced by anisotropic couplings, leading to higher critical temperatures, as emphasized by the recent discovery of superconductivity in nickelates7–10. Quantum simulators with ultracold atoms provide a versatile platform to engineer such couplings and to observe emergent structures in real space with single-particle resolution. Here we show, to our knowledge, the first signatures of individual stripes in a cold-atom Fermi–Hubbard quantum simulator using mixed-dimensional (mixD) settings. Increasing the energy scale of hole–hole attraction to the spin exchange energy, we access the interesting crossover temperature regime in which stripes begin to form11. We observe extended, attractive correlations between hole dopants and find an increased probability of forming larger structures akin to individual stripes. In the spin sector, we study correlation functions up to the third order and find results consistent with stripe formation. These observations are interpreted as a precursor to the stripe phase, which is characterized by interleaved charge and spin density wave ordering with fluctuating lines of dopants separating domains of opposite antiferromagnetic order12–14.

What are the hidden shortcomings of balance training research in older adults that prevent its transfer into practice? Scoping review

Background Although a lot of attention is paid to the flaws of balance training research in older adults, the low methodological quality and incomplete reporting of studies still limit the knowledge transfer between research and practice. These known shortcomings are considered also as barriers for creating recommendations for balance training in older adults. Despite the considerable efforts to improve the scientific quality of studies, such recommendations have not yet been formulated to date. Therefore, this scoping review aims (1) to analyze the literature that addresses balance training in older adults, (2) to identify and summarize gaps in the existing literature, and (3) to propose future research on this topic. Methods We focused on studies that evaluated the effect of balance training on balance control in apparently healthy older adults over 60 years of age. Results Out of 6910 potentially relevant studies, only 26 met the eligibility criteria. The identified shortcomings were as follows: missing a priori criteria for training session attendance and leisure-time physical activities, insufficiently described exercises and training load, and inappropriately chosen tests. Conclusions Among the shortcomings of the balance training research, the insufficiently described balance training program and inappropriately chosen tests can be considered the most important. For this reason, even with an excellently designed experiment, it is almost impossible for practitioners to apply the results of such studies into practice. Therefore, researchers should pay more attention to possible users of the acquired knowledge, which is more than desirable in the case of exercise programs for older adults.

Substance use and disordered eating risk among college students with obsessive-compulsive conditions

Purpose College students are at higher risk for problematic substance use and disordered eating. Few studies have examined the comorbid risks associated with OCD despite the increased prevalence of OCD among young adults. This study examined substance use and disordered eating risk associated with OCD conditions among college students and how this association may vary by sex/gender. Methods Data were from 92,757 undergraduate students aged 18–24 enrolled in 216 colleges between Fall 2021 and Fall 2022, from the American College Health Association-National College Health Assessment III. Regression models were used to estimate alcohol, cannabis, tobacco, and disordered eating risk among those with OCD related conditions compared to those without conditions, overall and by sex/gender, while adjusting for covariates and school clustering. Results Students with OCD conditions displayed a higher prevalence of substance use and disordered eating risks. In adjusted models, OCD conditions were associated with increased odds of moderate/high tobacco (aOR = 1.12, 95% CI 1.05, 1.21), cannabis (aOR = 1.11, 95% CI 1.04, 1.18), alcohol (aOR = 1.14, 95% CI 1.05, 1.24) and disordered eating risk (aOR = 2.28, 95% CI 2.13, 2.43). Analyses stratified by gender revealed cis-female students with OCD conditions were at increased risk for moderate/high risk alcohol (aOR = 1.18, 95% CI 1.08, 1.29), tobacco (aOR = 1.12, 95% CI 1.03, 1.22), cannabis (aOR = 1.13, 95% CI 1.06, 1.23) and disordered eating (aOR = 2.30, 9%% CI 2.14, 2.47). Among TGNC students, OCD conditions were associated with increased risk for moderate/high tobacco risk (aOR = 1.24, 95% CI 1.05, 1.48) and disordered eating (aOR = 2.14, 95% CI 1.85, 2.47). OCD conditions was only associated with disordered eating among male students (aOR = 2.34, 95% CI 1.93, 2.83). Discussion Young adult college students with OCD conditions exhibit a higher prevalence of medium/high risk alcohol, tobacco, and cannabis use and disordered eating compared to their counterparts without such conditions, even after adjusting for stress, depression, and anxiety.

“MoSpec”: A customized and integrated system for model development, verification and validation

Background and objective The growing availability of patient data from several clinical settings, fueled by advanced analysis systems and new diagnostics, presents a unique opportunity. These data can be used to understand disease progression and predict future outcomes. However, analysing this vast amount of data requires collaboration between physicians and experts from diverse fields like mathematics and engineering. Methods Mathematical models play a crucial role in interpreting patient data and enable in-silico simulations for diagnosis and treatment. To facilitate the creation and sharing of such models, the CNR-IASI BioMatLab group developed the “Gemini” (MoSpec/Autocoder) system, a framework allowing researchers with basic mathematical knowledge to quickly and correctly translate biological problems into Ordinary Differential Equations models. The system facilitates the development and computation of mathematical models for the interpretation of medical and biological phenomena, also using data from the clinical setting or laboratory experiments for parameter estimation. Results Gemini automatically generates code in multiple languages (C++, Matlab, R, and Julia) and automatically creates documentation, including code, figures, and visualizations. Conclusions This user-friendly approach promotes model sharing and collaboration among researchers, besides vastly increasing group productivity.

Effects of ketamine and propofol on muscarinic plateau potentials in rat neocortical pyramidal cells

Propofol and ketamine are widely used general anaesthetics, but have different effects on consciousness: propofol gives a deeply unconscious state, with little or no dream reports, whereas vivid dreams are often reported after ketamine anaesthesia. Ketamine is an N-methyl-D-aspartate (NMDA) receptor antagonist, while propofol is a γ-aminobutyric-acid (GABAA) receptor positive allosteric modulator, but these mechanisms do not fully explain how these drugs alter consciousness. Most previous in vitro studies of cellular mechanisms of anaesthetics have used brain slices or neurons in a nearly “comatose” state, because no “arousing” neuromodulators were added. Here we tested mechanisms of anaesthetics in rat medial prefrontal cortex (mPFC) slices after bath-applying the cholinergic agonist muscarine to partly mimic an “aroused-like” state, using whole-cell patch-clamp recordings from layer 2/3 pyramidal cells (L2/3PCs). According to leading theories of access consciousness and working memory, L2/3PCs are particularly important for these cognitive functions. We found that muscarine induced long-lasting depolarising plateau potentials (PPs) and spiking following brief depolarising current injections in the L2/3PCs. After 2 hours of pre-incubation with ketamine or propofol, the muscarine-induced PPs were altered in seemingly different ways: 3 μM propofol reduced the PPs and (significantly) spiking, whereas 20 μM ketamine seemed to enhance PPs and spiking (non-significantly). Brief wash-in of these drug concentrations failed to induce such effects, probably due to insufficient equilibration by diffusion in the slices. In contrast, pre-incubation with a high dose (100 μM) of ketamine suppressed the PPs and spiking. We discuss whether the apparently different effects on PPs may possibly be related to contrasting clinical effects: ketamine causing atypical anaesthesia with vivid, “psychedelic” dreaming while propofol causes less dreaming.

Conversational repairs on Reddit: Widely initiated but often uncompleted

Conversational repair has been proposed as a universal system for maintaining mutual understanding during social interactions. The repair system has been studied extensively in offline synchronous interactions (e.g., face-to-face, phone calls) and has been observed across cultures and languages. However, the prevalence of conversational repairs is unclear in online asynchronous text-based interactions. Online interactions are increasingly important for public deliberation, and it is therefore important to understand how conversational repairs manifest in different online contexts. To address this gap, we conducted two analyses of Other-initiated repairs in 25 English-language Reddit communities (subreddits), covering a diverse range of topics and communication norms. Analysis 1 examines the frequency of repair initiations across subreddits, finding them to be widespread (in every subreddit) and frequent (58.48% of interactions experience a repair initiation). Analysis 2 examines the emergence of repairs, finding that a repair initiation becomes increasingly likely the longer a comments thread progresses (Median time-to-repair = 6 turns). These results suggest that the prevalence and emergence of repair initiations in online interactions are comparable to offline contexts. However, we also find 44.80% of initiations receive no reply, precluding the possibility of a repair completion. Thus, conservatively, nearly half of the repair initiations in our data went uncompleted. This suggests that the online medium alters the way initiations are completed compared to offline interactions. We discuss the implications of this finding and avenues for future research.

Association between the triglyceride-glucose index and its combined obesity indicators and the risk of hypertension in middle-aged and older Chinese adults: A nationwide cross-sectional study

Objective This study aimed to explore the association between the triglyceride-Glucose Index (TyG) and its combination with obesity indicators in relation to the risk of hypertension among middle-aged and elderly individuals in China. Methods A total of 9,248 participants aged 45 and above were selected from the 2015 China Health and Retirement Longitudinal Study. Data on basic demographics, behavioral habits, medical history, physical examination results, and blood indicators were collected. The TyG and its obesity-related indicators, including TyG-BMI, TyG-WC, and TyG-WHtR were analyzed. These indicators were categorized into four groups based on quartiles, and the prevalence of hypertension within each group was assessed. Logistic regression analysis was conducted to evaluate the association between different TyG indices and obesity-related indicators with the risk of hypertension. Additionally, a restricted cubic spline model was employed to investigate the dose-response relationship between the TyG index, its combined obesity indicators, and the risk of hypertension. The receiver operating characteristic curve was utilized to determine the optimal predictive index for diagnosis. Results This study included a total of 9,248 middle-aged and elderly individuals aged 45 and above, comprising 4,274 males (46.21%), with an average age of 61.38 ± 9.28 years. The prevalence of hypertension exhibited an increasing trend as the quartiles of TyG, TyG-BMI, TyG-WC, and TyG-WHtR rose. After fully adjusting for covariates, comparisons between the first quartile (Q1) and the fourth quartile (Q4) of TyG, TyG-BMI, TyG-WC, and TyG-WHtR revealed that all these metrics at Q4 level were associated with an increased prevalence of hypertension. The odds ratios were as follows: TyG: OR = 1.80 (95% CI: 1.48–2.19); TyG-BMI: OR = 5.09 (95% CI: 4.33–5.98); TyG-WC: OR = 3.96 (95% CI: 3.38–4.65); TyG-WHtR: OR = 3.91 (95% CI: 3.33–4.60). A linear correlation was observed between TyG and the risk of hypertension (P for non-linearity = 0.2267), while non-linear correlations were noted between TyG-BMI, TyG-WC, and TyG-WHtR with the risk of hypertension (P for non-linearity < 0.001). The receiver operating characteristic curve indicated that TyG-WC demonstrated the highest diagnostic performance for hypertension, with an area under the curve of 0.642 (95% CI: 0.631–0.654). Conclusion As the levels of TyG, TyG-BMI, TyG-WC, and TyG-WHtR increased, the risk of hypertension among middle-aged and elderly individuals aged 45 and above in my country rises significantly. Among them, TyG-WC can be a predictor of hypertension in middle-aged and elderly people.
