Factors affecting early mobilization among critically Ill patients in Southern West Bank Hospitals

Introduction Early mobilization (EM) among critically ill patients is important to reduce complications such as ICU-acquired weakness and improve recovery outcomes. This study assessed factors influencing EM in Southern West Bank hospitals. Methods A cross-sectional study was conducted using the PERME ICU Mobility Score. Data were analyzed using SPSS to evaluate associations between mobility outcomes, demographics, BMI, and reasons for hospitalization. Inclusion criteria focused on critically ill patients admitted to ICUs for 2–5 days. Results Among 150 participants, the highest PERME ICU Mobility Score was observed in abdominal pain patients (mean = 28.0, SD = 0.0), reflecting minimal barriers, while respiratory patients scored lowest (mean = 6.9, SD = 6.9). No significant differences were found by age, gender, or BMI (p > 0.05). Seventy patients (46.7%) were awake/alert upon admission, while 39 (26%) were unresponsive. Pain or inability to determine pain levels was reported by 99 patients (66%). Conclusion Respiratory conditions and pain were critical barriers to EM. Demographic factors did not influence outcomes, but targeted pain management and condition-specific protocols are needed. The SD of 0.0 for abdominal pain patients suggests homogeneity in this subgroup, warranting further investigation.

Factors associated with early sexual debut among adolescents and youth in Mozambique: A geo-additive survival analysis of the Mozambique 2021 AIDS indicator survey

Engaging in sexual activity before the age of 15, known as early sexual debut, is a significant public health concern due to its association with increased risks of sexually transmitted infections, unintended pregnancies, and reduced educational achievements. This research investigates the factors influencing early sexual initiation among adolescents and youth aged 15 to 24 years in Mozambique, utilizing data of 5,283 individuals from the 2021 Mozambique AIDS indicator survey. Geoadditive models for censored time-to-event outcomes were employed to explore regional disparities and key determinants of the time to sexual debut, aiming to inform region-specific public health interventions and policies. The model was fitted using Poisson approximation within the framework of mixed model formulation for generalized additive models. The results revealed that approximately 80% of individuals have engaged in sexual activity, with 18.9% reporting having had sex before the age of 15. Among women, the percentage was 16.3%, while among men, it was 26.7%. Regional disparities were also observed, with the northern region exhibiting a higher prevalence of sexual debut before the age of 15 (26.7%). The median age at which individuals initiate sexual activity was estimated to be 16 years. After adjusting for significant covariates, the findings indicated a higher incidence of early sexual debut in the northern and southern regions of Mozambique, with notable gender differences. Age, occupation, alcohol consumption, illicit drug use, and living with parents were statistically significant factors associated with the age of sexual debut. These results highlight the need of addressing both individual behaviors and broader structural determinants—such as education and socio-economic conditions—to delay sexual initiation and reduce adverse health outcomes among Mozambican adolescents and youth. This study adds to the limited geospatial literature on sexual behavior in Mozambique.

Pitch characteristics of real-world infant-directed speech vary with pragmatic context, perceived adult gender, and infant gender

Children’s everyday language environments can be full of rich and diverse input, especially adult speech. Prosodic modifications when adults speak to infants are observed cross-culturally and are believed to enhance infant learning and emotion. However, factors such as what and why adults are speaking as well as speaker gender can affect the prosody of adults’ speech. This study asks whether prosodic modifications to infant-directed speech depend on perceived adult speaker gender, assigned infant gender, and the perceived pragmatic function of an utterance. We examined 3,607 adult speech clips from daylong home audio recordings of 60 North American, English-speaking, 3- to 20-month-old infants (28 female). Adult speakers used significantly more imperatives and questions and sang more frequently to infants than other adults. While infant-directed speech tended to have greater mean pitch and pitch modulation than adult-directed speech overall, these patterns were modulated, sometimes in complex ways, by pragmatic function, perceived adult gender, and infant gender. For example, we found that female-sounding adult speakers exhibited greater IDS-ADS mean pitch differences than male-sounding adult speakers when providing information or engaging in conversational niceties. An additional example is that male-sounding adults used higher pitch when singing to male infants compared to female infants. These findings invite further research on how individual, demographic, and situational factors affect speech to infants and possibly infant learning. The study’s pragmatic context tags are added to an existing open dataset of infant- and adult-directed speech.

Conditional knockout of C/EBPβ in epidermis results in dysregulated lipid biosynthesis and a defect in skin barrier function

CCAAT/enhancer binding protein-β (C/EBPβ) is a basic leucine zipper transcription factor that is abundantly expressed in epidermal keratinocytes of skin. In the present study, C/EBPβ epidermal specific conditional knockout (CKOβ) SKH1 mice were utilized to interrogate C/EBPβ’s role in lipid biosynthesis and skin barrier integrity. RNAseq data analysis and gene set enrichment analysis of RNA isolated from the epidermis of CKOβ and K5Cre control mice revealed that deletion of C/EBPβ in epidermis resulted in an enrichment of downregulated genes in gene sets associated with lipid metabolism. Further analysis showed the majority of differentially regulated genes were downregulated in gene sets related to the metabolism/biosynthesis of ceramides, fatty acids, phospholipids, sphingolipids, and cholesterol species in CKOβ epidermis. Ingenuity Pathway Analysis predicted inhibition of multiple pathways involving lipid biosynthesis. Lipidomic analysis of epidermis using advanced chemical separations and tandem mass spectrometry identified 470 individual lipids in epidermis with 165 significantly decreased and 82 significantly increased in CKOβ epidermis. The lysophospholipids were the most decreased class of lipids, and free fatty acids and ceramides important in barrier formation were also decreased. The sphingomyelin class of lipids was the most increased. High resolution mass spectrometry for cholesterol lipids revealed several cholesterol esters were also dysregulated in CKOβ epidermis. Finally, we assessed the functional consequences of the loss C/EBPβ on epidermal barrier function and found that basal permeability barrier function as measured by transepidermal water loss (TEWL) was impaired, with an approximate doubling of TEWL in CKOβ mice. These results indicate that C/EBPβ is a is a major regulator of the epidermal lipidome and the deletion of C/EBPβ in epidermis leads to a defect in skin barrier function.

Novel insights from comprehensive analysis: The role of cuproptosis and peripheral immune infiltration in Alzheimer’s disease

Background Cuproptosis is increasingly recognized as an essential factor in the pathological process of Alzheimer’s disease (AD). However, the specific role of cuproptosis-related genes in AD remains poorly understood. Methods Our first step was to obtain gene expression data from the GEO database and identify differentially expressed cuproptosis-associated genes (DECAGs) in AD. GO, KEGG, and GSEA analyses were then conducted on these genes. Subsequently, we attempted to classify AD patients by unsupervised clustering. Then, four machine-learning models were used to screen hub-genes from the DECAGs. We also explored the immune features of these genes and predicted target drugs. Molecular docking analysis was then performed on the predicted drugs and their corresponding hub-gene related proteins. Candidate markers were then validated by single-cell analysis and intracellular communication was investigated in a GEO scRNA-seq dataset. Lastly, we examined the expression levels of the hub-genes in peripheral blood cells using real-time quantitative PCR. Results 19 DECAGs were found in AD and the key biological processes and molecular functions associated with AD were further determined. Two subtypes of peripheral blood cells showed significant alternations in AD: Cluster1 and Cluster2. Five hub-genes including FDX1, GLS, PDK1, MAP2K1, and SOD1 were then screened out from the machine-learning study. All of the five hub-genes were significantly correlated with various immunocytes. We discovered compounds targeting hub-gene related proteins and forecasted multiple strong hydrogen bonding interactions between the picked predicted drugs and the target proteins by molecular docking analysis. Subsequently, in the single-cell analysis of AD peripheral blood, all hub-genes except SOD1 were found to be up-regulated in B cells, NK cells, and CD4+ T cells, possibly acting on the MIF pathway. Finally, we discovered that the levels of PDK1 expression in AD patients were remarkably upregulated, while FDX1 and GLS were significantly decreased using qPCR. Conclusion This study examined changes in intercellular communication between immune cells in the peripheral blood and identified five novel feature genes associated with cuproptosis in AD patients. These results facilitated a deeper understanding of the molecular mechanisms of AD and suggested novel therapeutic targets.

Societal attitudes and structural barriers in coaching para-athletes: A mixed-methods systematic review

Coaching athletes with disabilities is essential for promoting inclusivity and equity in sports. However, societal attitudes (e.g., stigmatization and ableism) and structural barriers (e.g., insufficient funding and limited access to adaptive equipment) persist as significant obstacles to their full participation and optimal performance. This systematic review addresses a critical gap in the existing literature by examining the complex interaction between societal and structural factors and their impact on coaching practice for athletes with disabilities. A mixed-methods approach was employed to synthesize data from 26 studies sourced from PubMed, Web of Science, SPORTDiscus, and Google Scholar. By integrating both qualitative and quantitative evidence, this review explores how societal biases and resource limitations converge to create systemic challenges that hinder coaching effectiveness. This review indicates that societal perceptions, including ableism and underestimation of athletic potential, in conjunction with structural barriers, such as inadequate facilities and funding, significantly undermine the effectiveness of coaching practices. Moreover, the findings emphasize the urgent need for comprehensive reforms in coach education and policy implementation with a particular focus on improving accessibility and ensuring the equitable distribution of resources. Ultimately, the evidence suggests that addressing these interconnected barriers is essential for fostering a more inclusive and equitable coaching environment for athletes with disabilities.

Association between body mass index and long-term all-cause mortality in critically ill patients without malignant tumors

Background The “obesity paradox” in certain diseases has been reported in previous studies. This study aimed to investigate the relationship between BMI and long-term mortality in all critically ill patients without malignant tumors who were admitted to the ICU. Methods Using the MIMIC-IV 2.2 database, we included all ICU admissions for patients without malignant tumors and categorized them into four groups based on the World Health Organization (WHO) obesity criteria. The relationship between BMI and 90-day, 180-day, and 1-year mortality was analyzed using univariate and multivariate Cox regression models, along with restricted cubic spline (RCS) models to account for potential non-linear associations. Results A total of 19,089 patients were included, with 90-day, 180-day, and 1-year mortality rates of 18.35%, 20.80%, and 23.96%, respectively. Overweight and obese patients exhibited significantly lower mortality rates compared to underweight and normal-weight individuals at all time points. After adjusting for confounders, higher BMI remained a protective factor for long-term mortality (HR 0.65–0.72, P < 0.001). RCS curves demonstrated a U-shaped relationship between BMI and mortality, and subgroup analyses confirmed the protective effect of higher BMI in different subgroups. Conclusion The “obesity paradox” may apply to critically ill patients without malignant tumors.

Demographic forecast modelling using SSA-XGBoost for smart population management based on multi-sources data

Population prediction could provide effective data support for social and economic planning and decision-making, especially for the sub-national population forecasting accurately. In addition to realizing efficient smart population management, this research focuses primarily on the combination model for forecasting demographic data based on machine learning. As to the higher error of population forecasts due to high population density and mobility, a dynamic monitoring method based on mobile communication big data such as mobile phone signals is proposed, combined with more structurally stable traditional statistical data, it forms a multi-source dataset that possesses both accuracy and real-time characteristics. In the study, the Extreme Gradient Boosting tree (XGBoost) model is used to identify the base model to create a reliable predictive model for population dynamic monitoring. The sparrow search algorithm (SSA) is investigated to obtain more reasonable parameters of XGBoost to improve forecast accuracy. The combination model is verified based on the data of the 6th and 7th national population census and mobile phone signal data in Hebei Province, obtained the predicted data for mortality and migration, categorized by age and gender, for the following year. Subsequently, the research compared the performance of different metaheuristic algorithms and various gradient-boosting machine-learning models on the dataset. The SSA-XGBoost model demonstrates a better prediction performance in the demographic data forecast with better R2 0.9984 and a lower mean absolute error of 0.0002 and a mean squared error of 6.9184. The results of the comparative experiments and cross-validation show that the proposed predictive model can effectively forecast the demographic data for sub-national regions to realize smart population management.

Mental health disorders among children with special health needs: A population-based cohort study using linked administrative data from Manitoba, Canada

Objective An estimated 15–22% of Canadian kindergarten-age children have a special health need (SHN), defined as a clinical diagnosis, a functional need requiring special accommodation at school, or a health condition leading to increased needs. Children with SHN may be more likely to experience mental health disorders than their peers without SHN, placing them at risk for further health and academic challenges. Our objective was to determine the odds of children with SHN identified in kindergarten being diagnosed with a mental health disorder by age 16. Methods In this retrospective cohort study using population-based, linked administrative data, we identified children with SHN born 1995–2020 in Manitoba, Canada, and enrolled in kindergarten from 2006–2011. The SHN designation is derived from the Early Development Instrument. We measured prevalence of common childhood mental health disorders (ADHD, mood/anxiety disorders, conduct disorders) in children with SHN to age 16. Using binary logistic regressions, we calculated crude odds ratios (OR) for children with vs. without SHN being diagnosed with a mental health disorder, then adjusted for age, sex, and neighbourhood-level income. Results Among 42,766 children, 13.8% had a SHN designation in kindergarten. Among these, 41.0% were diagnosed with a mental health disorder by age 16. The odds of a mental health diagnosis by SHN category were: special needs designation in kindergarten (OR 1.75, 95%CI 1.53–2.01); learning impairment (OR 1.61, 95%CI 1.39–1.86); behavioural impairment (OR 3.27, 95%CI 2.87–3.72); and emotional impairment (OR 2.01, 95%CI 1.75–2.32). Children with SHN (vs. none) had higher odds of a mental health disorder if they had 1 + impairment (OR 1.67, 95%CI 1.50–1.85). Adjusting for sociodemographic characteristics did not change the estimates. Conclusions The study highlights important kindergarten predictors of future mental health disorders in children, which should be used to inform preventive and supportive strategies for children with SHN and help generate wider mental health supports in schools.

Motives of Chinese foreign direct investment in Africa: With regulation effects of institutional quality

This article analyzes the motives of Chinese foreign direct investment (FDI) in Africa and the impact of the institutional quality of the host country for the period between 2003 and 2022 from both static and dynamic perspectives by using the OLS, PPML, and GMM methods. The results show that: China has obvious market-, and efficiency-seeking motives, but weak resource- and strategic asset-seeking motives. There is a tipping point for market- and efficiency-seeking motives. Good institutional quality of the host country (region) facilitates FDI expansion. Investment inertia and factor endowments affect long-term FDI. We also examine the moderating effects of institutional quality and the Belt and Road Initiative (BRI) and found that: the host country’s (region’s) institutional environment optimization makes market size more attractive and labor costs less attractive. The BRI itself has no obvious influence on Chinese investment in Africa, while the impact of market size and labor costs became more significant after 2013. Hence, when selecting investment locations, Chinese enterprises should prioritize the host country’s (region’s) market size, labor costs, and institutional quality. Additionally, they should utilize the moderating effect of institutional quality to mitigate the disadvantages associated with higher labor costs.

Effect of high-intensity anaerobic exercise on electrocortical activity in athletes and non-athletes

Aim The present study aims to verify the information processing in athletes through electroencephalography, analyze cortical areas responsible for cognitive functions related to attentional processing of visual stimuli, and investigate motor activity’s influence on cognitive aspects. Additionally, we aimed to analyze the acute effect of physical exercise after the high-intensity anaerobic effort, evaluating how a short-duration Wingate test influences cortical activity and attentional processing immediately following exertion. Materials and methods The sample consisted of 29 subjects, divided into an experimental group (n = 13 modern pentathlon athletes) and a control group (n = 16 non-athletes). We collected the electrocortical activity before and after the Wingate Anaerobic Test. During the electrophysiological measures, the volunteers performed a saccadic eye movement paradigm. They also performed cognitive tasks, resting heart rate, and anthropometric measurements. Results A mixed ANOVA was applied to analyze the statistical differences between groups (athletes and control) and moments (before and after exercise) for F3, F4, P3, and P4 electrodes during rest one and task (pre-stimulus GO). There was an interaction for the group vs. moment factors in F3 [F = 17,129; p = 0,000; η² = 0.512], F4 [F = 22,774; p = 0,000; η² = 0.510], P3 [F = 11,429; p = 0,001; η² = 0.405], and P4 electrodes [F = 18,651; p = 0,000; η² = 0.379]. We found the main effect for group factors in the frontal and parietal electrodes of the right hemisphere (F4 and P4) and a main effect of the moment factor on the frontal (F3 and F4) and parietal (P3 and P4) electrodes. There was an interaction between the group vs. moment factors for the reaction time. The groups were different in Peak Power (Watts/kg), Average Power (Watts/kg), Fatigue Index (%), and Maximum Power (ms). Conclusions We identified chronic effects of exercise training on the cortical activity of modern pentathlon athletes, read-through differences in absolute alpha power, and acute effects of a high-intensity exercise session for athletes and non-athletes for electrocortical and behavioral responses.
