Differential risk of cardiovascular complications in patients with type-2 diabetes mellitus in Ghana: A hospital-based cross-sectional study
Aim To characterize clinically relevant subgroups of patients with type-2 diabetes mellitus (T2DM) based on adiposity, insulin secretion, and resistance indices. Methods A cross-sectional study was conducted at Eastern Regional Hospital in Ghana from July to October 2021 to investigate long-term patients with T2DM. To select participants, a systematic random sampling method was employed. Demographic data was collected using a structured questionnaire and fasting blood samples were taken to measure glycemic and lipid levels. Blood pressure and adiposity indices were measured during recruitment. The risk of cardiovascular disease (CVD) was defined using Framingham scores and standard low-density lipoprotein thresholds. To analyze the data, k-means clustering algorithms and regression analysis were used. Results The study identified three groups in female patients according to body mass index, relative fat mass, glycated hemoglobin, and triglyceride-glucose index. These groups included the obesity-related phenotype, the severe insulin resistance phenotype, and the normal weight phenotype with improved insulin resistance. Among male patients with T2DM, two groups were identified, including the obesity-related phenotype with severe insulin resistance and the normal weight phenotype with improved insulin sensitivity. The severe insulin resistance phenotype in female patients was associated with an increased risk of high CVD (OR = 5.34, 95%CI:2.11–13.55) and metabolic syndrome (OR = 7.07; 95%CI:3.24–15.42). Among male patients, the obesity-related phenotype with severe insulin resistance was associated with an increased intermediate (OR = 21.78, 95%CI:4.17–113.78) and a high-risk CVD (OR = 6.84, 95%CI:1.45–32.12). Conclusions The findings highlight significant cardiometabolic heterogeneity among T2DM patients. The subgroups of T2DM patients characterized by obesity and/or severe insulin resistance with or without poor glycemic control, have increased risk of CVD. This underscores the importance of considering differences in adiposity, insulin secretion, and sensitivity indices when making clinical decisions for patients with T2DM.

Enhanced hunger games search algorithm that incorporates the marine predator optimization algorithm for optimal extraction of parameters in PEM fuel cells
Abstract This article introduces a novel optimization approach to improve the parameter estimation of proton exchange membrane fuel cells (PEMFCs), which are critical for diverse applications but are challenging to model due to their nonlinear behavior. The proposed method, HGS-MPA, enhances the Hunger Games Search (HGS) algorithm by integrating Marine Predator Algorithm (MPA) operators, significantly boosting its exploitation capabilities and convergence rate. The effectiveness of HGS-MPA was validated on three commercial PEMFC datasets: 250-W stack, BCS 500-W, and NedStack PS6, using the Sum Squared Error (SSE) as the performance metric. Experimental results highlight that HGS-MPA achieves minimum fitness values of 0.33770, 1.31620, and 0.01174 for the respective datasets, outperforming other state-of-the-art algorithms. These findings underscore the method’s potential for accurate PEMFC parameter estimation, offering enhanced performance and reliability.
Efficacy of acupuncture in treating chemotherapy-related cognitive impairment in breast cancer patients: A systematic review and meta-analysis protocol
Background Chemotherapy-related cognitive impairment (CRCI) is a common adverse effect of chemotherapy in breast cancer patients, with a high incidence that significantly impacts treatment adherence and quality of life. Currently, there is no definitive and effective treatment for CRCI. Research indicates that acupuncture may serve as a promising intervention for CRCI. This study aims to assess the effectiveness of acupuncture in treating CRCI. Methods This study is guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). From the time of database construction to October 2024, we will search seven literature databases, three clinical study registry databases, and three sources of gray literature. The restricted languages are English and Chinese. We will use the Participants-Interventions-Comparators-Outcome-Conclusion-Study Design (PICOS) criteria to identify included studies and the RoB2 tool to assess the quality of the included studies. Data synthesis and meta-analysis will be performed using RevMan 5.3. Discussion The results of this study will clarify the potential benefits of acupuncture in the treatment of CRCI and address existing disagreements about its efficacy. Furthermore, the findings will assist clinicians in making informed decisions regarding the treatment of CRCI. Trail registration PROSPERO registration number: CRD42024568093.

Dry eye disease treatment improves subjective quality-of-life responses in patients with AMD, independent of disease stage
Purpose To determine the impact of severity of age-related macular degeneration (AMD) on subjective treatment response in patients treated for dry eye disease. Methods A total of 203 eyes diagnosed with evaporative dry eye disease (DED) due to meibomian gland dysfunction were treated using the LipiFlow or MiBoFlo systems. From this cohort, 40 eyes with stable dry AMD (early, intermediate, or late stages) were included. Each participant completed the Ocular Surface Disease Index (OSDI) and Standard Patient Evaluation of Eye Dryness Questionnaire (SPEED) before treatment and at a 6-month follow-up. Changes in questionnaire scores were analyzed using one-way analysis of variance (ANOVA) to assess differences between AMD severity groups. Results Improvement in SPEED and OSDI scores, including vision related OSDI scores were observed across all AMD stages, with no significant differences between groups (p<0.05). Conclusion Managing DED improved quality of life (QOL) in patients with AMD, regardless of retinal disease severity. This highlights the importance of treating coexisting ocular surface conditions to enhance patient outcomes, even in the presence of significant maculopathy.

3D echocardiography derived reference values and determinants of left ventricular twist and torsion from the population-based STAAB cohort study
Abstract Left ventricular (LV) rotational function parameters provide in-depth information about LV mechanical function as well as prognostic information. Using three-dimensional (3D) echocardiography, we identified determinants of LV “twist” and “torsion”, and established reference values using a large population-based cohort. 3D echocardiography images were recorded in n = 2803 subjects within the prospective STAAB cohort study investigating a representative age- and sex-stratified sample of residents of the city of Würzburg, aged 30–79 years, without history of heart failure. Valid 3D image analysis was performed in n = 1831 (65.3%) subjects (mean age 57 ± 11 years, 49.3% women). Using general linear models, we identified determinants of LV twist and torsion: there was a positive association between LV rotational parameters and age, female sex, and blood pressure but a negative association with body weight. From a subset of 479 apparently healthy individuals exhibiting no cardiovascular (CV) risk factors or CV disease (mean age 52 ± 10 years, 56.4% women), we derived reference percentiles for twist and torsion. LV rotation is determined by a complex interplay of sub-endocardial and sub-epicardial fibers which might be affected differentially by potential risk factors. We found a differential association with respective determinants as LV rotational parameters increased with age and with higher blood pressure but decreased with higher body weight. Further research is needed to elucidate these associations in more detail and to determine the additional information contributed by twist and torsion. To facilitate respective attempts and to set an individual’s results in relation to a population-based reference, we derived normal values for twist and torsion from a sub-collective of healthy individuals.
Machine learning identifies the association between second primary malignancies and postoperative radiotherapy in young-onset breast cancer patients
Background A second primary malignant tumor is one of the most important factors affecting the long-term survival of young women with breast cancer (YWBC). As one of the main treatments for breast cancer YWBC patients, postoperative radiotherapy (PORT) may increase the risk of second primary malignancy (SPM). Methods Machine learning components, including ridge regression, XGBoost, k-nearest neighbor, light gradient boosting machine, logistic regression, support vector machine, neural network, and random forest, were used to construct a predictive model and identify the risk factors for SPMs with data from the Surveillance, Epidemiology and End Results. Multivariate logistic regression analysis was used to assess the risk of SPM associated with PORT. The cumulative incidence of SPMs was determined by competing risk regression analysis. Results Among the 44223 YWBC patients included in our study, 3017 developed SPMs. Among all the clinical characteristics, PORT was the most common SPM. YWBC patients receiving PORT had significantly greater risks of second primary solid malignancies (SPSMs, RR = 1.61), including breast cancer (RR = 1.89), lung cancer (RR = 2.12) and thyroid cancer (RR = 1.48), but not second primary hematologic malignancies (RR = 1.32; 0.94–1.88). SPSMs were more common in YWBC individuals who were black, had a lower median household income and had fewer lymph nodes examined. Additionally, we developed a prediction nomogram with an area under the curve of 0.75 to assess the likelihood of developing SPMs. Conclusion YWBC patients receiving PORT had a greater risk of developing SPSMs (thyroid, lung, and breast cancer), indicating the necessity of long-term surveillance of these patients. Standard adjuvant PORT should not be recommended for breast cancer patients with favorable histology and a low risk of relapse.

Analysis of the kinematic pattern of free throw by wheelchair basketball athletes: a systematic review
Athletes are stratified into classes that range from 1.0 to 4.5 points subdivided at intervals of 0.5 points in the Wheelchair Basketball (WCB) modality. The discussion regarding the non-utilization of quantitative data based on functional classification (FC) in WCB has become considerably important for research due to its impact on athletes’ careers. The aim of this systematic review was to verify the kinematic patterns of the free throw performed by WCB athletes of distinct FC. After researching four online databases, only 7 out of 68 studies fulfilled the inclusion criteria. High-class players (classes 3.0 – 4.5) presented a throw pattern, with a greater angle on the shoulder joint, on the angular speed of the wrist, and the height of the ball throw; low-class athletes (classes 1.0 – 2.5) exhibited a pushing pattern, with greater speed and throw angles, greater angular speed at the shoulder and elbow joints. Thus, inter-individual differences allow for the stratification of players into distinct classes and serve as relevant tools for increasing accuracy and performance despite different disabilities. Further studies are necessary to explore the associated patterns for each FC.

Assessment of contamination of natural waters with radionuclides and heavy metals the case of Karabulak creek at the Semipalatinsk Test Site
This paper presents research data on water contamination levels of artificial radionuclides and heavy metals in Karabulak Creek at the Semipalatinsk Test Site. The uranium isotopic composition has been studied (235U and 238U). Priority contaminants and the most ‘contaminated’ sections of the Karabulak creek were identified, and water quality-related indicators were also estimated. The environmental situation around Karabulak Creek at the «Degelen» site—at which underground nuclear tests have been conducted—was found to be unique and attributed to the combined effect of man-made radiation and natural factors and, in particular, to geochemical features of the region.
