Impact of local therapy in metastatic renal cell carcinoma during medical treatment in a retrospective analysis
Abstract Current guidelines on treatment of metastatic renal cell carcinoma (mRCC) suggest an emerging role of local therapy (LT). Still there is a lack of data which patients may benefit from additional LT once medical treatment (MT) is initiated. We retrospectively aim to characterize LT in patients with mRCC who underwent LT while receiving MT. 315/401 mRCC patients were eligible, thereof 163 (51.7%) received LT during MT (LT( +)), while 152 (48.3%) received only MT (LT(-)). Radiotherapy (49.1%) and surgery (41.7%) were the most frequently administered LT modalities. Overall survival (OS) was not superior in LT( +) vs. LT(-) (35.9, (95%-CI [confidence interval]: 29.8–42.0) vs. 20.3, (95%-CI: 10.3–30.3) months, log-rank p = 0.117). However, in a subgroup analysis the duration of MT prior to initiation of LT (≤ 6 months 24.1 (95%-CI: 18.6–29.6) vs. > 6 months: 43.0 (95%-CI: 32.2–36.2) months, log-rank p = 0.005) and the type of progression (oligoprogression: 44.0 (95%-CI: 31.5–56.5) vs. systemic progression: 29.6 (95%-CI: 23.4–35.8) months, log-rank p = 0.03) were associated with improved OS. We present the largest analysis of LT during MT. Our study has enhanced our understanding of LT utilization in mRCC after MT is already initiated. Ultimately, the inclusion of LT could improve OS in selected patients receiving MT.

Serum zonulin level in autistic children and its relation to severity of symptoms a case-control study
Abstract Evidence suggests a possible link between Autism spectrum disorder and gut permeability, specifically as indicated by serum zonulin levels. However, limited studies examine this connection to symptom severity, especially in Egypt. Assessing serum zonulin level in children with autism and its relation to the severity of symptoms. In Mansoura University Hospital’s pediatric psychiatry outpatient clinics, case-control research was carried out with children with Autism diagnoses and age- and gender-matched typically developing controls. The Childhood Autism Rating Scale was used to gauge the severity of the symptoms, and the Enzyme-Linked Immunosorbent Assay (ELISA) was used to detect serum zonulin levels. Serum zonulin levels were considerably higher in children with Autism than in the control group (p < 0.05). Additionally, there was a positive correlation between higher zonulin levels and the degree of autism symptoms as determined by CARS scores (r = X, p < 0.05). Children with severe Autism had the highest zonulin levels, according to subgroup analysis, which suggests a possible connection between gut permeability and the intensity of symptoms. This study emphasizes how serum zonulin may serve as a biomarker for intestinal permeability in kids with Autism and how it may be related to the intensity of symptoms. These results highlight the need for more investigation into the gut-brain axis as a potential therapeutic target for Autism. Addressing gut permeability may provide new ways to lessen the intensity of symptoms and enhance results for children with Autism.

Abstract One of the problems facing our society today is managing hazardous waste. Expired pharmaceuticals are a type of waste that is not recycled in any way but is instead burned for disposal. These pharmaceutical drugs have been shown to have corrosion-inhibiting properties for various metals and corrosive solutions. The current study examined the corrosion tendency of 1040 carbon steel (1040CS) under acidic pH (1 M HCl) by using an unused, expired Thiocolchicoside (ETC) drug as a possible corrosion inhibitor was studied by using the weight loss (WL), electrochemical impedance spectroscopy (EIS), and potentiodynamic polarisation (PP) tests, and. It was observed that the increase in ETC doses led to the rise in corrosion IE (%) up to 93.3, 90.7, and 91.4%, as shown by EIS, PDP, and WL techniques, respectively. However, inhibition efficiency diminished with rising temperatures, declining from 91.83% at 298 K to 78.63% at 318 K (all at 400 ppm). The results obtained high Rct during electrochemical processes, which reduced Icorr and, in turn, the ETC molecule strong adsorption onto the 1040CS surface. The PP data showed that this ETC decreased the corrosion current density by a mixed-type mechanism. SEM, AFM, and FTIR surface studies demonstrated the development of a protective layer at the CS surface. DFT-based quantum chemical indices made further understanding of the inhibitory mechanism possible. Results gained for all techniques used are in good agreement.
