Neck circumference cutoff point as a predictor of metabolic syndrome in Brazilian rural workers

Neck circumference (NC) is a predictive measure for the diagnosis of Metabolic Syndrome (MS). The aim of the present study was to establish cutoff points for NC as a predictor of the presence of MS in Brazilian rural workers, based on the MS components according to the IDF and NCEP-ATP III criteria. This is a cross-sectional study carried out with rural workers in the municipality of Santa Maria de Jetibá, in the state of Espírito Santo, Brazil. The ROC curve was calculated and the cutoff points for predicting the risk of developing MS were stipulated from the NC, identified by the area under the curve, using different methods of criteria for determining MS. Sensitivity, specificity, positive and negative predictive values and Youden index were applied. The significance level adopted was 5%. The cutoff points were different for males, resulting in 39.550 cm (AUC 0.832) according to the NCEP-ATP III criterion and 39.125 cm (AUC 0.888) according to the IDF criterion. For women, the cutoffs were similar, resulting in a single cutoff of 34.725 cm (AUC 0.862 for NCEP-ATP III and 0.849 for IDF). The cutoff points defined for men and women for NC showed good sensitivity and specificity for predicting MS in the studied population. The NC measurement proved to be a simple, low-cost and accurate measure for assessing this morbidity in Brazilian rural workers.

Spatio-temporal distribution and socioeconomic inequality of low birthweight rate in China from 1992 to 2021 and its predictions to 2030

This paper aims to investigate the trend, spatio-temporal distribution, and socioeconomic inequality of the low birthweight rate (LBWR) in China from 1992 to 2021 and to project the LBWR to 2030. We performed a secondary analysis of data from the China Health Statistics Yearbook. LBWR refers to the ratio of the number of infants born with a birth weight less than 2,500 grams to the number of live births in a given year. We used joinpoint regression models to estimate LBWR trends from 1992 to 2021 for the whole country and from 2002 to 2021 for the three regions (eastern, central, and western regions) and each province. The slope index of inequality (SII) and relative index of inequality (RII) were calculated for each year from 2002 to 2021 based on provincial data. LBWR increased from 2.52% (1992) to 3.70% (2021), and the average annual percentage change (AAPC) (95% confidence interval [CI]) was 1.35% (0.22%, 2.49%) in China. The overall LBWR from 2002 to 2021 was greatest in the Eastern region, but LBWR had the fastest increase in the Western region, with an AAPC (95% CI) of 3.15% (2.59%, 3.12%). There were spatio-temporal differences in the LBWR and trends between provinces. The SII and RII increased linearly from -0.15 and 0.94 to 0.53 (B = 0.035%, p < 0.001) and 1.16 (B = 0.011, p < 0.01), respectively, over the past 20 years. The results of the ARIAM model showed that the National LBWR will be increasedfrom 3.70% in 2021 to 5.28% in 2030. The LBWRs in the eastern, central and western regions in 2030 will be 4.93%, 6.02% and 5.82%, respectively. National and local governments must prioritize disadvantaged groups to mitigate the rapid prevalence of LBWR, reduce regional disparities, and improve perinatal and infant health and health equity in China.

Incidence rate of total knee arthroplasties in eleven European countries: Do they reach a plateau?

Background From several decades, the evolutions of the Incidence Rate (IR) of Primary Knee Arthroplasties are continuously increasing worldwide and have been widely studied in several countries. Some recent works have highlighted the fact that the IR is following a sigmoid curve composed of an exponential growth followed by a linear phase and finished by a plateau. Our objective is to assess the IR evolution of eleven European countries, representing thus a large proportion of this continent, regarding this sigmoid. Methods IRs of primary knee arthroplasties for Austria, Denmark, Finland, France, Germany, Hungary, Italy, Poland, Spain, Sweden, and the United Kingdom between 2005 and 2019 were retrieved from the EUROSTAT database. Several regression models were fitted to each country’s IRs: Poisson, linear, asymptotic, logistic, and Gompertz regression. For each country and each model, the RMSE (Root Mean Square Error) and R2 were calculated and used to estimate their position with respect to this sigmoid curve. Results The best regression models for knee arthroplasties varied following countries. Logistic and Gompertz regressions had the lowest RMSE and R2 values for Austria, Denmark, Germany, Sweden, and the UK. Hungary, Italy, and Poland favored the Poisson regression model. Finland and Spain presented difficulties in determining the optimal model (linear or Poisson), while France faced challenges in choosing between logistic, Gompertz, and linear regression. Conclusion In conclusion, the growth dynamics of IR differ across European countries. Some countries seem to have already reached a plateau and will therefore experience slight growth in the future.

Assessment tools for transition readiness in adolescents with inflammatory bowel disease: A scoping review

Background Assessing the level of transition readiness in adolescents with inflammatory bowel disease is crucial; however, standardized research tools are lacking. This study aimed to map transition readiness assessment tools for adolescents with inflammatory bowel disease and determine their suitability. Methods A literature review following the Arksey and O’Malley scoping review methodology was conducted. By using appropriate key terms, literature on transition readiness assessment tool searches were conducted in the CNKI, WanFang, SinoMed, Pubmed, Cochrane Library, Web of Science, and CINAHL databases, with a reference search. The retrieval period was from the establishment of the databases to January 2024. Results A total of 2561 studies were obtained through a preliminary search, and 5 references were obtained as retrospective references. Finally, 21 studies were selected for this review. In total, 20 transition readiness assessment tools were identified. Qualitative findings were grouped into five thematic areas: descriptive characteristics of reviewed articles, development procedures, design, psychometric properties, and cohort characteristics for validity testing of transition readiness assessment tools. Conclusions The most appropriate way to assess the transition readiness of adolescents with inflammatory bowel disease is to select an assessment tool that is most suitable for individual needs, accompanied by a comprehensive patient evaluation. Despite some flaws in the methodology, TRM is currently the most suitable assessment tool, and more population studies are needed to validate it.

MYO7A is required for the functional integrity of the mechanoelectrical transduction complex in hair cells of the adult cochlea

Myosin-VIIA (MYO7A) is an unconventional myosin responsible for syndromic (Usher 1B) or nonsyndromic forms of deafness in humans when mutated. In the cochlea, MYO7A is expressed in hair cells, where it is believed to act as the motor protein tensioning the mechanoelectrical transducer (MET) channels, thus setting their resting open probability ( P o ). However, direct evidence for this unique role for an unconventional myosin in mature hair cells is lacking. Here, we show that MYO7A has a distinct role in hair cells, being crucial for the structural integrity of hair bundles. Postnatal deletion of Myo7a leads to 87 to 96% reduction in MYO7A from hair cells by postnatal day 20 (P20), without affecting hearing function. During the following week, mice showed progressive decline in both hearing function and MET current amplitude in hair cells without affecting the resting P o and calcium sensitivity of the MET channel. Hair-bundle stiffness was normal at P20 but halved at P30, despite it having a normal staircase morphology and tip links. The reduction of MYO7A in the stereocilia (>87%) increased their vulnerability to sound-induced damage, with significantly more hearing loss and hair bundle deterioration than in control mice. RNA-sequencing identified a downregulation of several stereociliary genes in the Myo7a -deficient cochlea, indicating the presence of indirect compensatory mechanisms. This study reveals that mature hair cells seem to use a MYO7A-independent mechanism to maintain the resting P o of the MET channels. Instead, MYO7A is essential for maintaining the structural and functional integrity of the hair bundles.

Feasibility and acceptability for LION, a fully remote, randomized clinical trial within the VA for light therapy to improve sleep in Veterans with and without TBI: An MTBI2 sponsored protocol

Sleep-wake disturbances frequently present in Veterans with mild traumatic brain injury (mTBI). These TBI-related sleep impairments confer significant burden and commonly exacerbate other functional impairments. Therapies to improve sleep following mTBI are limited and studies in Veterans are even more scarce. In our previous pilot work, morning bright light therapy (MBLT) was found to be a feasible behavioral sleep intervention in Veterans with a history of mTBI; however, this was single-arm, open-label, and non-randomized, and therefore was not intended to establish efficacy. The present study, LION (light vs ion therapy) extends this preliminary work as a fully powered, sham-controlled, participant-masked randomized controlled trial (NCT03968874), implemented as fully remote within the VA (target n = 120 complete). Randomization at 2:1 allocation ratio to: 1) active: MBLT (n = 80), and 2) sham: deactivated negative ion generator (n = 40); each with identical engagement parameters (60-min duration; within 2-hrs of waking; daily over 28-day duration). Participant masking via deception balanced expectancy assumptions across arms. Outcome measures were assessed following a 14-day baseline (pre-intervention), following 28-days of device engagement (post-intervention), and 28-days after the post-intervention assessment (follow-up). Primary outcomes were sleep measures, including continuous wrist-based actigraphy, self-report, and daily sleep dairy entries. Secondary/exploratory outcomes included cognition, mood, quality of life, circadian rhythm via dim light melatonin onset, and biofluid-based biomarkers. Participant drop out occurred in <10% of those enrolled, incomplete/missing data was present in <15% of key outcome variables, and overall fidelity adherence to the intervention was >85%, collectively establishing feasibility and acceptability for MBLT in Veterans with mTBI.

Restart uncertainty relation for monitored quantum dynamics

We introduce a time-energy uncertainty relation within the context of restarts in monitored quantum dynamics. Previous studies have established that the mean recurrence time, which represents the time taken to return to the initial state, is quantized as an integer multiple of the sampling time, displaying pointwise discontinuous transitions at resonances. Our findings demonstrate that the natural utilization of the restart mechanism in laboratory experiments, driven by finite data collection time spans, leads to a broadening effect on the transitions of the mean recurrence time. Our proposed uncertainty relation captures the underlying essence of these phenomena, by connecting the broadening of the mean hitting time near resonances, to the intrinsic energies of the quantum system and to the fluctuations of recurrence time. Our uncertainty relation has also been validated through remote experiments conducted on an International Business Machines Corporation (IBM) quantum computer. This work not only contributes to our understanding of fundamental aspects related to quantum measurements and dynamics, but also offers practical insights for the design of efficient quantum algorithms with mid-circuit measurements.

Revealing the impact of tadalafil-loaded proniosomal gel against dexamethasone-delayed wound healing via modulating oxido-inflammatory response and TGF-β/Macrophage activation pathway in rabbit model

A serious challenge of the chronic administration of dexamethasone (DEX) is a delay in wound healing. Thus, this study aimed to investigate the potential of Tadalafil (TAD)-loaded proniosomal gel to accelerate the healing process of skin wounds in DEX-challenged rabbits. Skin wounds were induced in 48 rabbits of 4 groups (n = 12 per group) and skin wounds were treated by sterile saline (control), TAD-loaded proniosomal gel topically on skin wound, DEX-injected rabbits, and DEX+TAD-loaded proniosomal gel for 4 weeks. The optical photography, transmission electron microscopy, in vitro release profile, and stability studies revealed the successful preparation of the selected formula with good stability. DEX administration was associated with uncontrolled oxido-inflammatory reactions, suppression in immune response in skin wounds, and consequently failure in the healing process. TAD-loaded proniosomal gel-treated rabbits manifested a marked enhancement in the rate of wound closure than control and DEX groups (p < 0.05). The TAD-loaded proniosomal gel successfully antagonized the impacts of DEX by dampening MDA production, and enhancing total antioxidant capacity, coupled with modulation of inflammatory-related genes, inducible nitric oxide synthase, tumor necrosis factor-alpha, interleukin-1β, and matrix metalloproteinase 9, during all healing stages (p < 0.05). This was in combination with significant amplification of immune response-related genes, CD68 and CD163 (p < 0.05). Moreover, the histopathological, Masson’s Trichrome-stain, and immune-histochemical studies indicated a successful tissue recovery with the formation of new blood vessels in groups treated with TAD-loaded proniosomal gel, as manifested by well-organized collagen fibers, upregulation of transforming growth factor β1, and vascular endothelial growth factor immune expression in skin tissues (p < 0.05). Overall, the topical application of TAD-loaded proniosomal gel is useful in improving the delayed wound healing linked to DEX therapy via regulating the release of inflammatory/macrophage activation mediators and enhanced antioxidant capacity, angiogenesis, and vascularity.

The pleasurable urge to move to music is unchanged in people with musical anhedonia

In cognitive science, the sensation of “groove” has been defined as the pleasurable urge to move to music. When listeners rate rhythmic stimuli on derived pleasure and urge to move, ratings on these dimensions are highly correlated. However, recent behavioural and brain imaging work has shown that these two components may be separable. To examine this potential separability, our study investigates the sensation of groove in people with specific musical anhedonia. Individuals with musical anhedonia have a blunted ability to derive pleasure from music but can still derive pleasure from other domains (e.g., sex and food). People with musical anhedonia were identified as those with scores in the lower 10% of scores on the Barcelona Musical Reward Questionnaire, but who had no deficits in music perception, no symptoms of depression, average levels of physical and social anhedonia, and sensitivity to punishment and reward. We predicted that if the two components of groove are separable, individuals with musical anhedonia would experience lower levels of derived pleasure but have comparable ratings of wanting to move compared to controls. Groove responses were measured in an online study (N = 148) using a set of experimenter-generated musical stimuli varying in rhythmic and harmonic complexity, which were validated in several previous studies. Surprisingly, we found no significant differences in groove response between individuals with musical anhedonia (n = 17) and a matched control group (n = 17). Mediation analyses for the anhedonia sample found that wanting to move ratings fully mediated the effect of rhythmic and harmonic complexity on pleasure ratings. Taken together, these results indicate that the urge to move may compensate for the blunted pleasure sensation in those with musical anhedonia. More generally, these results suggest that the urge to move is a primary source of pleasure in the groove response.

Activity impairment, work productivity, health-related quality-of-life, and costs associated with uncomplicated urinary tract infection among females in Germany

Uncomplicated urinary tract infections (uUTIs) represent a sizable healthcare burden with a substantial negative impact on patients’ health-related quality-of-life (HRQoL). However, the HRQoL impact of uUTI from the patient perspective is under-represented in research. A cross-sectional online survey assessed activity impairment and work productivity, HRQoL, costs, and treatment satisfaction of female patients with uUTIs in the US; the current study applies this earlier methodology to Germany to provide a European perspective. We examined: activity impairment and work productivity using Activity Impairment Assessment (AIA), and Work Productivity and Activity Impairment (WPAI) questionnaires, respectively; HRQoL using a modified Short Form-36, and participants completed the treatment satisfaction questionnaire for medication. Participants (≥ 12 years) self-reported a uUTI treated with oral antibiotics in the last 60 days, had no evidence of complicated UTI, and were recruited via a consumer panel. In the patient (N = 200) survey, bladder pain (70%), dysuria (69%), and frequent urination (66%) were the most common uUTI symptoms reported. Activities frequently affected by uUTI were sexual activity (56%) and exercise (48%), resulting in an overall AIA score of 8.2 indicating that activities among patients were impaired ‘most of the time’ during their recent uUTI. Absenteeism was 29% (of a 40-hour work week); presenteeism, 34%; and overall work impairment, 41%. Indirect costs due to impact of uUTI were €7.62/hour/participant, representing a loss for the employer. The lowest HRQoL scores were for mental (44%) and physical components (43%); emotional and physical functioning scores were 71% and 70%, respectively. Mean direct costs of €20.10 for medical treatments and €11.30 for other treatment costs were reported. Overall, most participants were satisfied with the uUTI treatment (76% satisfaction score). Results demonstrate the patient-reported burden of uUTI for patients and employers, with activities and ability to work impacted, resulting in a notable indirect burden in Europe.

Time evolution of a pumped molecular magnet—A time-resolved inelastic neutron scattering study

Introducing an experimental technique of time-resolved inelastic neutron scattering (TRINS), we explore the time-dependent effects of resonant pulsed microwaves on the molecular magnet Cr 8 F 8 Piv 16 . The octagonal rings of magnetic Cr 3+ atoms with antiferromagnetic interactions form a singlet ground state with a weakly split triplet of excitations at 0.8 meV. A 4.6 tesla field was applied to tune the splitting between two members of the triplet excited level | 1 ⟩ ↔ | 2 ⟩ to resonance with 105 GHz (0.434 meV) microwaves. The time-dependent occupations of the ground state | 0 ⟩ , lower lying levels | 1 ⟩ and | 2 ⟩ , and higher energy states | λ ≥ 3 ⟩ were extracted during and after 20 s long microwave pulses incident along the (101) direction of a Cr 8 F 8 Piv 16 crystal held at 1.9 K. At significantly elevated spin temperatures, we found underpopulation relative to thermal equilibrium of | 2 ⟩ and spin-lattice thermalization time scales ranging from 1.6(2) s to 5.7(2) s depending on the power level. This contrasts with the relaxation time τ 1 ( T → 0 ) = 27 ( 5 ) μ s inferred for | 2 ⟩ from in situ Electron Spin Resonance measurements. By probing a broad range of excited states during intense microwave pumping, TRINS thus provides a first view of long lived excited states in a molecular antiferromagnet.

Avoiding catastrophic overfitting in fast adversarial training with adaptive similarity step size

Adversarial training has become a primary method for enhancing the robustness of deep learning models. In recent years, fast adversarial training methods have gained widespread attention due to their lower computational cost. However, since fast adversarial training uses single-step adversarial attacks instead of multi-step attacks, the generated adversarial examples lack diversity, making models prone to catastrophic overfitting and loss of robustness. Existing methods to prevent catastrophic overfitting have certain shortcomings, such as poor robustness due to insufficient strength of generated adversarial examples, and low accuracy caused by excessive total perturbation. To address these issues, this paper proposes a fast adversarial training method—fast adversarial training with adaptive similarity step size (ATSS). In this method, random noise is first added to the input clean samples, and the model then calculates the gradient for each input sample. The perturbation step size for each sample is determined based on the similarity between the input noise and the gradient direction. Finally, adversarial examples are generated based on the step size and gradient for adversarial training. We conduct various adversarial attack tests on ResNet18 and VGG19 models using the CIFAR-10, CIFAR-100 and Tiny ImageNet datasets. The experimental results demonstrate that our method effectively avoids catastrophic overfitting. And compared to other fast adversarial training methods, ATSS achieves higher robustness accuracy and clean accuracy, with almost no additional training cost.

A phase transition in diffusion models reveals the hierarchical nature of data

Understanding the structure of real data is paramount in advancing modern deep-learning methodologies. Natural data such as images are believed to be composed of features organized in a hierarchical and combinatorial manner, which neural networks capture during learning. Recent advancements show that diffusion models can generate high-quality images, hinting at their ability to capture this underlying compositional structure. We study this phenomenon in a hierarchical generative model of data. We find that the backward diffusion process acting after a time t is governed by a phase transition at some threshold time, where the probability of reconstructing high-level features, like the class of an image, suddenly drops. Instead, the reconstruction of low-level features, such as specific details of an image, evolves smoothly across the whole diffusion process. This result implies that at times beyond the transition, the class has changed, but the generated sample may still be composed of low-level elements of the initial image. We validate these theoretical insights through numerical experiments on class-unconditional ImageNet diffusion models. Our analysis characterizes the relationship between time and scale in diffusion models and puts forward generative models as powerful tools to model combinatorial data properties.

Health status correlates of malnutrition diagnosed based on the GLIM criteria in older Polish adults—Results of the PolSenior 2 study

Introduction Older individuals are at risk of malnutrition resulting from chronic diseases-related body and muscle mass reduction. In turn, nutritional deficiencies may enhance catabolic processes, leading to accelerated aging and comorbidity, thus creating a vicious cycle. Our study aimed to assess the prevalence of malnutrition using the Global Leadership Initiative on Malnutrition (GLIM) criteria and to determine the health correlates of malnutrition in a representative sample of community-dwelling older adults. Methods We used the GLIM criteria to diagnose malnutrition in 5,614 participants of the PolSenior2 study. The PolSenior2 study was a population-based survey designed to assess the medical, psychological, social, and economic characteristics of community-dwelling older adults. Results Malnutrition was diagnosed in 13.4% of the participants using the GLIM criteria. Results of multiple logistic regression showed that the risk of depression [OR 4.18, p<0.001], peptic ulcer disease [OR 2.73, p<0.001], past stroke [OR 1.71, p<0.001], cognitive impairment [OR 1.34, p = 0.015], and chronic pain [OR 1.23, p = 0.046] were independent correlates of malnutrition. Conclusion Due to the high risk of malnutrition, special attention should be paid to individuals in late old age. Suspected malnutrition should also be considered in people at risk of depression, with peptic ulcer disease, past stroke, and cognitive impairment. Chronic pain should also prompt the diagnosis for malnutrition.

PhiSiCal-Checkup: A Bayesian framework to validate amino acid conformations within experimental protein structures

As structural biology and drug discovery depend on high-quality protein structures, assessment tools are essential. We describe a new method for validating amino-acid conformations: “PhiSiCal ( ϕ ψ χ al) Checkup.” Twenty new joint probability distributions in the form of statistical mixture models explain the empirical distributions of dihedral angles ω , ϕ , ψ , χ 1 , χ 2 , … of canonical amino acids in experimental protein structures. Marginal and conditional probability distributions for subsets of dihedral angles are derived from these joint mixture models. Together, these distributions are employed to measure rapidly the information-theoretic “favorability” of any proposed experimental protein structure. The inferred statistical models and measures overcome several shortcomings and afford improvements over the current state of the art in amino-acid conformation verification. Experimental comparisons are made against current protein conformation verification software. In a number of examples, we pick up outliers that are invisible to current methods. We also calculate, as part of verification, the sensitivity of favorability to small changes in a proposed structure accounting for the precision of coordinates. In some cases a near neighbor of a proposed amino-acid conformation may be either less or more favorable. This raises the question, is the current reliance on fixed “thresholds” for validation a good thing? Woody species composition and diversity of agroforestry homegardens along altitudinal gradient in southwest Ethiopia

Homegarden agroforestry systems that integrate trees with agricultural practices are usually valued for the conservation of farm biodiversity. Despite the system having a significant conservation role, litle is known on woody species composition and diversity following the elevation belt of southwest Ethiopia. A complete enumeration of 72 homegardens (24 each from altitudinal gradient) was purposively selected for woody species inventory. A total of 55 woody species belonging to 31 families and 45 genera were recorded. Of which, 56.4% of woody species are indigenous and two are endemic to Ethiopia. Family Fabaceae was the most represented family with eight species. The highest species richness (42) was observed at high altitudes followed by 39 species at middle and 31 species at low altitudes but no significant difference between them. Species richness significantly (P < 0.001) increased with increasing wealth status. The overall richness distribution was 46, 40, and 27 across rich, medium, and poor wealth classes respectively. Shannon diversity index differed significantly between poor and rich households. Pearson correlation result shows a strong positive and significant correlation between richness and wealth status. The mean woody species density was 89.06±9.25 and 1236.22±131.42 per garden and hectare, respectively. Stem density was significantly higher (P < 0.001) in wealthier farms. We found that, in southwestern Ethiopia wealthier agroforestry farms can support more woody species diversity in homegardens.

Development and validation of the Iranian Minimum Data Set for Epidermolysis Bullosa: A mixed method approach

Minimum Data Set (MDS) enables integration in data collection, uniform data reporting, and data exchange across clinical and research information systems. The current study was conducted to determine a comprehensive national MDS for the Epidermolysis Bullosa (EB) information management system in Iran. This cross-sectional descriptive study consists of three steps: systematic review, focus group discussion, and the Delphi technique. A systematic review was conducted using relevant databases. Then, a focus group discussion was held to determine the extracted data elements with the help of contributing multidisciplinary experts. Finally, MDSs were selected through the Delphi technique in two rounds. The collected data were analyzed using Microsoft Excel 2019. In total, 103 data elements were included in the Delphi survey. The data elements, based on the experts’ opinions, were classified into two main categories: administrative data and clinical data. The final categories of data elements consisted of 11 administrative items and 92 clinical items. The national MDS, as the core of the EB surveillance program, is essential for enabling appropriate and informed decisions by healthcare policymakers, physicians, and healthcare providers. In this study, a MDS was developed and internally validated for EB. This research generated new knowledge to enable healthcare professionals to collect relevant and meaningful data for use. The use of this standardized approach can help benchmark clinical practice and target improvements worldwide.

Optimizing power grids: A valley-filling heuristic for energy-efficient electric vehicle charging

The expansion of electric vehicles (EVs) challenges electricity grids by increasing charging demand, thereby making Demand-Side Management (DSM) strategies essential to maintaining balance between supply and demand. Among these strategies, the Valley-Filling approach has emerged as a promising method to optimize renewable energy utilization and alleviate grid stress. This study introduces a novel heuristic, Load Conservation Valley-Filling (LCVF), which builds on the Classical and Optimistic Valley-Filling approaches by incorporating dynamic load conservation principles, enabling better alignment of EV charging with grid capacity. We conducted a comprehensive analysis of the heuristic across five EV charging scenarios. In both the Original and Flexible scenarios, LCVF reduced energy demand by up to 10.65%, demonstrating its adaptability and effectiveness. Notably, in the 24-hour Availability scenario, LCVF achieved a reduction of over 20% in energy demand compared to CVF. These findings indicate that LCVF could play a crucial role in enhancing real-world EV charging infrastructure, boosting energy efficiency and grid stability. By integrating DSM strategies like LCVF, energy grids can better accommodate renewable energy sources, promoting more sustainable operations.

Effectiveness of menstrual hygiene management training to enhance knowledge, attitude, and practice among adolescents in Sindhupalchowk, Nepal

Background Menstrual Health (MH) knowledge, attitude and practice (KAP) are highly affected by access to information regarding menstruation. Despite being included in the school curriculum, Sexual and Reproductive Health (SRH) education is often not delivered in practice. School-based educational interventions have been shown to be effective in promoting MH. Methods A school-based study was conducted in Indrawati rural municipality of Sindhupalchowk district in Nepal. 427 participants (175 boys and 252 girls), aged 11–13, completed a questionnaire evaluating MH KAP before receiving a structured training module on MH provided by experienced trainers from Putali Nepal using the Menstrupedia tool. The questionnaire was repeated one month after the training. Pre and post intervention scores were compared to determine the effect of the intervention. Focus group discussions were also conducted to understand the perceptions of participants toward SRH teaching. Association of independent socio-demographic with dependent variables knowledge and attitude towards menstrual health were analyzed using MANOVA test. The Wilcoxon signed-rank test was used to compare the median outcome of the pre and post-test attitude and knowledge. The maximum possible score was 6 for MH knowledge. The total attitude score ranged 14 to 70. Results The median knowledge score increased by 1 point (p = <0.001) and the median attitude score by 5 points (p = <0.001), one month after delivery of the intervention. Higher knowledge scores were significantly associated with Hindu religion, female gender, higher father’s literacy, and mothers in an informal occupation on multivariate analysis. Higher attitude scores were significantly associated with Hindu religion while lower attitude scores were associated with a mother in an informal occupation. Conclusion The Menstrupedia comic educational intervention improved knowledge and attitude towards menstruation among Nepali adolescents. A scale-up of the Menstrupedia based intervention would significantly change knowledge and attitude towards menstruation in Nepali adolescents.

Hypermethylation at 45S rDNA promoter in cancers

The ribosomal genes (rDNA genes) encode 47S rRNA which accounts for up to 80% of all cellular RNA. At any given time, no more than 50% of rDNA genes are actively transcribed, and the other half is silent by forming heterochromatin structures through DNA methylation. In cancer cells, upregulation of ribosome biogenesis has been recognized as a hallmark feature, thus, the reduced methylation of rDNA promoter has been thought to support conformational changes of chromatin accessibility and the subsequent increase in rDNA transcription. However, an increase in the heterochromatin state through rDNA hypermethylation can be a protective mechanism teetering on the brink of a threshold where cancer cells rarely successfully proliferate. Hence, clarifying hypo- or hypermethylation of rDNA will unravel its additional cellular functions, including organization of genome architecture and regulation of gene expression, in response to growth signaling, cellular stressors, and carcinogenesis. Using the bisulfite-based quantitative real-time methylation-specific PCR (qMSP) method after ensuring unbiased amplification and complete bisulfite conversion of the minuscule DNA amount of 1 ng, we established that the rDNA promoter was significantly hypermethylated in 107 breast, 65 lung, and 135 colon tumour tissue samples (46.81%, 51.02% and 96.60%, respectively) as compared with their corresponding adjacent normal samples (26.84%, 38.26% and 77.52%, respectively; p < 0.0001). An excessive DNA input of 1 μg resulted in double-stranded rDNA remaining unconverted even after bisulfite conversion, hence the dramatic drop in the single-stranded DNA that strictly required for bisulfite conversion, and leading to an underestimation of rDNA promoter methylation, in other words, a faulty hypomethylation status of the rDNA promoter. Our results are in line with the hypothesis that an increase in rDNA methylation is a natural pathway protecting rDNA repeats that are extremely sensitive to DNA damage in cancer cells.
