Mortality trends in metabolic syndrome and colorectal, endometrial, and breast cancers in the US (1999–2020).

Abstract

e13582 Background: Metabolic syndrome (MS) increases the risk of colorectal, endometrial, and breast cancers, contributing to significant mortality. This study examines U.S. mortality trends (1999–2020), focusing on demographic and regional disparities. Methods: CDC WONDER data for adults 25+ from 1999 to 2020 were analyzed. Age-adjusted mortality rates (AAMRs) per million were calculated by gender, race/ethnicity, age, and region. Trends were assessed using Joinpoint regression, with average annual percentage change (AAPC), 95% confidence intervals (CIs), and p-values. Results: MS-related cancers caused 231,437 deaths, with AAMRs per million rising from 27.91 in 1999 to 57.7 in 2020 (AAPC: 2.86% p < 0.000001). Gender: Females had higher mortality (175,568 deaths; AAMR: 61.93) than males (55,869 deaths; AAMR: 28.53) with AAPCs of 3.08% ( p < 0.000001) for females and 2.78% (p < 0.000001) for males. Age: Elderly (65+) had significantly higher AAMRs (215.77/million) than adults 25–64 (7.72/million). Elderly AAMRs peaked in 2020 at 245. Adults 25–64 saw the highest AAMR in 2020 at 12.2/million. AAPCs: 4.71% ( p < 0.000001) for adults and 2.53% (p < 0.000001) for the elderly. Race: Blacks had the highest AAMR (79.45), while Hispanics had the steepest increase (AAPC: 4.78% p < 0.000001). AAMRs also rose for Whites (46.53), American Indians (42.02), and Asians (29.58). Geography: The South had the highest mortality (81,463 deaths; AAMR: 53.38) and the steepest increase (AAPC: 4.38%; p < 0.000001). The Midwest followed (AAMR: 53.38), while the Northeast had the lowest (46.04). Conclusions: Mortality from MS-related cancers rose significantly from 1999 to 2020, with disparities. Targeted interventions are needed for at-risk populations, especially females, elderly, Blacks, and residents of the South. Strengthened public health efforts focusing on prevention and MS management may help reduce this growing burden.
