Clinical outcomes for anemic patients with early-onset breast cancer: An analysis of the National Inpatient Sample.

e23102 Background: Despite recent global decreases in overall cancer incidence, the incidence of early-onset breast cancer (EOBC) is steadily increasing globally. Limited data is available on the comorbid correlations for this unfortunately expanding population. We sought to examine the national inpatient sample database to describe in-hospital outcomes among patients with anemia and EOBC. Methods: Data were extracted from the National Inpatient Sample (NIS) Database for 2019 and 2020. The NIS was searched for hospitalizations of adult patients with EOBC, defined as all-cause breast cancer in patients 50 years old or younger. We then examined the outcomes of patients who were noted to be anemic. Multivariate logistic was used to adjust for confounders. The primary outcome was inpatient mortality and secondary outcomes were annotated accordingly. SPSS software was used for statistical analysis, and all results were powered to p < 0.001. Results: This study included 10,764 patients with EOBC, of which 3597 (33.4%) were identified as anemic. Multivariate regression showed that anemic patients with EOBC had higher inpatient mortality (OR 1.663, CI 1.536-1.800, p < 0.001). On secondary analysis, anemic patients with EOBC were more likely to have thrombocytopenia (OR 2.655, CI 2.433-2.898), hypertension (OR 3.197, CI 2.751-3.715), acute renal failure (OR 2.074, CI 1.926-2.232), pancreatitis (OR 1.511, CI 1.279-1.784), mitral regurgitation (OR 1.500, CI 1.282-1.755), pericarditis (OR 1.636, CI 1.471-1.820), intracranial hemorrhage (OR 1.429, CI 1.225-1.665), Crohn’s disease (OR 1.500, CI 1.235-1.821), celiac disease (OR 1.267, CI 1.004-1.598), severe liver disease (OR 1.781, CI 1.715-1.850), lymphoma (OR 1.571, CI 1.214-2.034), hypothyroidism (OR 1.411, CI 1.225-1.625), obstructive sleep apnea (OR 1.642, CI 1.499-1.800), all-cause arrhythmias (OR 1.661, CI 1.514-1.823), all-cause sepsis (OR 1.812, CI 1.738-1.889), all-cause coagulopathy (OR 1.975, CI 1.790-2.178), all-cause heart failure (OR 1.952, CI 1.766-2.157), and all-cause stroke (OR 1.696, CI 1.541-1.867). Conclusions: In this nationally representative, population‐based retrospective cohort study, patients with anemia and EOBC were associated with higher mortality and worse outcomes.
