Analysis of niraparib-induced changes in glucose levels during three-month therapy in normoglycemic patients with ovarian cancer.

e17575 Background: Poly(ADP-ribose) polymerase inhibitors (PARPis) are promising drugs for ovarian cancer (OC) patients, but their safety profile is still not well defined. In a previous study, the authors showed that olaparib-induced hyperglycaemia occurred in more than 50% of the OC patients after three months of therapy [1]. However, there is a lack of data on whether this adverse effect can occur after other PARPis such as niraparib and whether it can be defined as a class effect. The aim of the study was a retrospective analysis of glycaemia during three-month niraparib therapy in normoglycemic patients with OC. Methods: The study was conducted on 20 OC patients (mean [SD] age, 63.7 [16.6] years) with normoglycemia (4.8-5.5 mmol/l). The fasting glucose (FG) levels were measured before and after 3 months of niraparib treatment. The severity of observed hyperglycemia or hypoglycemia was assessed using the Common Terminology Criteria for Adverse Events (CTCAE v5.0). Results: In the OC patients there was statistically significant difference between the initial FG, recorded before niraparib treatment, and FG after three months of therapy with this PARPi (p=0.0270). Twelve patients (60%) showed an increase in FG (min-max; 0.1-1.54 mmol/l), 6 of whom had confirmed glycemic values characteristic of a pre-diabetic state (5.6–6.5 mmol/l). Six patients (30%) had a decrease in FG (min-max; 0.1-0.5 mmol/l). Conclusions: In view of the observed changes in glycemia during niraparib therapy, it seems essential to recommend monitoring of this parameter during therapy with this PARPi, as in the case of olaparib. 1.
