Abstract

On behalf of the International Liaison Committee on Resuscitation (ILCOR) Pediatric Life Support Task Force. Background: Death and neurodisability secondary to hypoxic ischaemic brain injury are important complications following return of circulation after cardiac arrest. Active temperature management to avoid hyperthermia, and targeted hypothermia are options for post resuscitation care. Ongoing assessment of optimal active temperature management, including temperature target, duration, speed of temperature change, methods and impact of delay in initiation, remain key questions to guide clinician decision making and best practice. Method: We are conducting a combined evidence review program, searching MEDLINE, EMBASE and Cochrane databases for literature of children receiving active temperature management after cardiac arrest. Part 1: A systematic review: will be performed to explore the efficacy of a) a specific temperature target and b) any temperature target, and c) duration of targeted temperature management&Part 2: A scoping review will be performed to map the literature available on additional elements of temperature targeted management; including a) speed/rate of reaching temperature target, b) delays in initiation of active temperature target management, c) speed/rate of rewarming in patients actively controlled at hypothermic temperature and d) methods of controlling temperature through the active temperature management process. Outcomes include survival or survival with favorable neurological outcome as defined by the Pediatric Core Outcome Set for Cardiac Arrest. Results: At the time of abstract submission, screening of published literature is ongoing and the work is planned for completion in time for the AHA ReSS conference. Full presentation of data will be available. Conclusion and future direction: The results of the two part review will inform consensus on science and treatment recommendations for ILCOR and resuscitation guidelines.
