Abstract 4371204: Nationwide Consensus of 800 Experts on Statin Combinations with Ezetimibe and Clopidogrel for Lipid Management and Cardiovascular Risk Reduction

Abstract

Introduction: The AngioStat initiative aimed to establish expert consensus on statin combination strategies involving ezetimibe and clopidogrel, focusing on their clinical relevance in optimizing lipid control and cardiovascular risk reduction. Methods: The AngioStat consensus engaged 800 experts in dyslipidemia management, representing over 25,000 cumulative years of clinical experience. Participants evaluated 9 clinical statements related to statin combination therapy with ezetimibe and clopidogrel using a 5-point Likert scale. Data were analyzed using GraphPad v10.3.0, with strong consensus defined as a weighted score exceeding 100. Results: The perception mapping revealed strong expert consensus on key lipid and antiplatelet strategies in cardiovascular care. Highest agreement was seen for clopidogrel plus statin in long-term post-PCI protection (score: 113.0) and TAVR patients on statin with SAPT (score: 111.9). Cumulative LDL-C exposure as a major atherosclerotic risk also scored high (score: 109.1). Other prioritized strategies included SAPT in high bleeding risk patients (score: 108.8), genetic lipid risk profiling (score: 107.7), and ezetimibe-based LDL-C lowering (score: 107.1). The overall mean response scores were: agree: 75±9.1 (95% CI 68 to 82), strongly agree: 32±12 (95% CI 22 to 41), p<0.0001 Conclusion: The AngioStat consensus confirms expert alignment on combining statins with ezetimibe and clopidogrel for optimized cardiovascular care, supporting their integration in long-term lipid and antiplatelet management strategies
